甘氨双唑钠对鼻咽癌放射治疗的增敏作用

摘　　要：目的探讨甘氨双唑钠(CMNa)配合放射治疗对局部晚期鼻咽癌的临床疗效和毒副反应。方法40例鼻咽癌患者随机分为两组,治疗组(放疗+CMNa)20例,对照组(单纯放疗)20例,两组患者临床资料相似,放疗方法相同,均用常规分割,原发灶和颈转移灶剂量7000～7400cGy/35～37f/7～7.5w。治疗组放疗同时使用CMNa每次800mg/m2,每周3次,从放疗开始连续使用至放疗结束。结果治疗组和对照组患者治疗结束时肿瘤原发灶的完全消退率(CR率)分别为95.0%、85.0%,放疗剂量分别为4905.0cGy、5835.6cGy,颈转移灶CR率分别为89.5%、88.9%,放疗剂量分别为4500.0cGy、5342.8cGy,两组患者原发灶、颈转移灶达CR时的放射剂量不同,有统计学差异。毒副反应两组比较无统计学差异,未见神经系统毒性。结论CMNa合并放疗对鼻咽癌有增敏作用,可提高近期疗效,降低疗效达CR时所需的放射剂量,无明显毒副反应。Objective To evaluate the radiosensitivity and toxicity of CMNa on local advanced nasopharyngeal carcinoma. Methods 40 patients with local advanced nasopharyngeal carcinoma were divided into two groups randomly： treatment group （n = 20） and control group （n = 20）. The patients in treatment group were intravenous dripped CMNa 800mg/m^2 three times weekly during whole treatment period by nasopharyngeal carcinoma. Patients in control group were treated with radiotherapy alone. The method of radiotherapy in control group was as same as that in treatment group. The doses for primary lesion and neck metastatic lesion were 7 000-7 400cGy/35-37f/7-7. 5w. Results For primary lesion, the complete regression （CR） rates in treatment group and in control group were 95. 0% and 85. 0% respectively, and the doses by which the effects had reached CR were 4905.0cGy and 5835.6cGy respectively. That doses of neck metastatic lesion in the two groups were 4500. 0cGy and 5342. 8cGy respectively. In the two groups, there were significante difference in the doses by which the effects had reached CR, while no significante difference in toxicity. The toxicity of nerve system was not observed in the two groups. Conclusion Our study suggests that CMNa should enhance radiosensitivity on nasopharyngeal carcinoma, increase the short-term therapeutic response and decrease the radiotherapy dose without obvious toxicity.
