核糖核酸酶抑制因子联合化疗栓塞治疗大鼠Walker-256移植性肝癌的实验研究

摘  要:目的 评价核糖核酸酶抑制因子(RI)联合化疗栓塞(TACE)治疗大鼠Walker-256移植性肝癌的疗效。材料与方法45只Walker-256荷瘤大鼠于移植后第14d随机分成RI+碘油栓塞组(RI栓塞组,10只);5-氟尿嘧啶(5-Fu)+碘油栓塞组(TACE组,12只);RI+5-Fu+碘油栓塞组(联合栓塞组,13只);对照组(生理盐水组,10只)。各组分别经肝动脉左支注入栓塞剂和药物,RI用量为200U,超液态碘油为0.4ml,5-Fu为20mg/kg体重,生理盐水为0.4ml。用MR分别于栓塞后第7d、第13d、第18d测量肿瘤横径并计算体积。于术后第18d检测肝转移结节数。观察RI的系统毒性。结果 栓塞后7~18d各治疗组肿瘤体积均明显小于对照组(P〈0.05),以联合栓塞组体积最小。在栓塞后第13d,联合栓塞组肿瘤的体积组明显小于RI栓塞组(P〈0.05);第18d TACE组亦明显小于RI栓塞组(P〈0.05)。肝脏转移结节数各治疗组明显少于对照组(P〈0.05),联合栓塞组明显少于RI栓塞组和TACE组(P〈0.05)。RI无明显系统毒性。结论 联合栓塞与单纯TACE相比,可明显抑制大鼠Walker-256移植性肝癌的生长和减少肝转移,提高了TACE的疗效。经肝动脉给药后,RI对Wistar大鼠无明显系统毒性。Objective To evaluate the effect of combination with ribonuclease inhibitor(RI) and chemoembolization(TACE) on the rat walker 256 hepatuma model. Materials and Methods The 45 rats of walker 256 hepatuma model were randomly divided into RI embolization, TACE, combination embolization and control group on 14 day after the tumors were implanted. Hepatic artery perfusion with RI 200U and lipiodol 0.4ml in RI embolization group ( n = 10) ; 5Fu 20mg/kg and lipiodol 0.4 in TACE group ( n = 12) ; RI 200U, 5Fu20mg/kg and lipiodol 0.4 in combination embolization ( n = 13) ; saline solution 0.4ml in control group ( n = 10). The sizes of tumors were evaluated with MR on day 7, 13 and 18 after treatment. The numbers of liver metastasis node were assessed in every group on day 18 after treatment. The toxicity of RI was also evaluated. Results The average tumor volume of every treatment group was smaller than that of control group( P 〈 0.05), there was the smallest volume in combination embolization group. The average tumor volume in combination embolization group was significant smaller than that of RI embolization group on day 13 after treatment ( P 〈 0.05), and The average tumor volume in TACE group was significant smaller than RI embolization group on day 18 after treatment (P 〈 0.05) too, The number of liver metastasis node in every treatment group was significant decreased compared with in control group (P 〈 0.05), the numbers of liver metastasis node in combination embolization group were significant decrease compared with RI embolization group and TACE group (P 〈 0.05 ). There was no signifinant different of the toxicity of RI between artery infusion group and control group ( P 〈 0.05). Conclusion The effect of inhibit tumor growth and liver metastasis is enhanced by combination embolization with RI and TACE on the rat walkar 256 hepatuma model, there is no system toxicity on RI.
