脑脊液置换及鞘内注药治疗蛛网膜下腔出血

摘　　要：【目的】研究采用脑脊液置换及鞘内注药治疗蛛网膜下腔出血(SAH)的疗效及临床应用价值。【方法】将本院2001年12月至2004年12月收治的70例SAH患者作为治疗组应用脑脊液置换及鞘内注药;逆时间选取2001年12月以前采用传统保守治疗的65例SAH病人作为对照组。【结果】治疗组病人头痛持续时间较对照组明显缩短(P<0.01,χ2=50.835),脑血管痉挛、脑积水、脑梗死等并发症发生率明显低于对照组(均P<0.01),治疗组病死率较对照组明显下降(P<0.05,χ2=3.923),但采用脑脊液置换及鞘内注药并不能有效防止再出血并发症的发生,亦不增加发生再出血的几率。【结论】脑脊液置换及鞘内注药治疗蛛网膜下腔出血有较高的临床应用价值,适合临床普及应用。[Objective]To explore the clinic effect of cerebrospinal fluid （CSF） replacement and intrathecal injection on subarachnoid hemorrhage （SAH）. [Methods]Seventy SAH patients were taken as the treatment group with CSF replacement and intrathecal injection from December 2001 to December 2004, while 65 SAH patients who were treated with conventional methods before December 2001 were selected as the control group. [Results]The time of headache in treatment group was shorter than that in control group （ P 〈0.01）. The treatment of CSF replacement and intrathecal injection could markedly reduce the occurrence of complications, such as cerebrovaseular spasm, hydroeerebrum and cerebroinfaret （ P 〈 0.01）. The probability of death declined obviously with this method（ P 〈0.05）. But, this method could not prevent or cause rehemorrhage. [Conclusion]The treatment of SAH with CSF replacement and intrathecal injection has great clinical meanings and can be generalized.
