80岁以上胰十二指肠切除术体会

摘　　要：目的探讨80岁以上高龄病人行胰十二指肠切除术的可行性。方法回顾性分析我院1999—2003年收治的7例80岁以上病人胰十二指肠切除术资料。结果 7例高龄病人并发症发生3例(42.9%),其中胃潴留3例次(42.9%),肺部感染2例次(28.6%),胰瘘、消化道出血、ARDS、切口感染均1例次(14.3%)。结论高龄不是手术禁忌证;加强围手术期处理和胰腺专业组手术是手术安全性和规范性的保证;对高龄病人强调术前或术中病理诊断。Objective To investigate the feasibility of pancreaticoduodenectomy in patients with periampullary cancer of an age over 80 yrs. Methods The clinical data of 7 patients with an age over 80 yrs receiving pancreaticoduodenectomy in our hospital from April 1999 to December 2003 were retrospectively analyzed. Results Postoperative complications occurred in 3 patients （42.9%）. The complications were gastroparalysis in 3 cases, pulmonary infection in 2, pancreatic leakage in 1, gastrointestinal bleeding in 1, ARDS in 1 and wound infection in 1. Conclusions Pancreaticoduodenectomy is suitable to be performed in aged people. The intensive perioperative care and excellent operative technique of the operators are important for operative security and standardization. Preoperative or intraoperative pathological examination should be emphasized for senile patients.
