β-连环蛋白和E-钙黏蛋白在结直肠癌中的表达及其意义

摘　　要：背景:结直肠癌是我国常见的消化道恶性肿瘤之一。近年研究发现β-连环蛋白(catenin)及其相关基因和黏附分子钙黏蛋白(cadherin)的改变在结直肠癌的发生、发展过程中发挥重要作用。目的:探讨β-catenin和E-cadherin的表达与结直肠癌生物学行为的关系及其作为患者预后指标的可能性。方法:应用免疫组化方法检测60例结直肠癌组织及其相应癌旁黏膜和其中24例有淋巴结转移者转移淋巴结组织中β-catenin和E-cadherin的表达。术后对患者进行随访以了解生存率。结果:结直肠癌组织中β-catenin和E-cadherin的异常表达率显著高于相应癌旁黏膜,并随肿瘤分化程度的降低和Dukes分期的进展而增高。β-catenin和E-cadherin表达异常者术后2年生存率显著低于表达正常者(P<0.05)。结论:β-catenin和E-cadherin表达异常是判断结直肠癌浸润转移的良好指标,也可作为近期预后指标之一。Background： Colorectal carcinoma is one of the common gastrointestinal malignant tumors in our country. It has been demonstrated that changes of β-catenin and its related genes as well as adhesion molecule cadherins play important roles in the development and progression of colorectal carcinoma. Aims： To investigate the relevance of expressions of β- catenin, E-cadherin, and the biological behavior of colorectal carcinoma, as well as their possibility of being taken as prognostic markers. Methods： Immunohistochemical studies of β-catenin and E-cadherin expressions were conducted in 60colorectal carcinoma specimens, 60corresponding paracancerous tissues and 24 metastatic lymph node specimens. The patients were followed-up postoperatively for the survival rate. Results： The proportions of abnormal expressions of β- catenin and E-cadherin in colorectal cancerous tissues were significantly higher than those in the corresponding paracancerous tissues, and the abnormality rates increased with decreased degree of tumor differentiation and increased Dukes staging. Concordantly, the postoperative two-year survival rate in those with abnormal expression of β-catenin and E-cadherin were much lower than those with normal expression （P〈0.05）. Conclusions： Abnormal expression of β-catenin and E-cadherin can be promising markers for predicting presence of invasion and metastasis and can be prognostic predictors of colorectal carcinoma.
