摘  要:目的:观察降纤酶治疗早期缺血性脑卒中前后血浆中血管内皮生长因子(vascular endothelialgrowth factor VEGF)、内皮素(endothelin ET-1)、P-选择素(P-selectin)和血小板活化因子(platelet activating fac-tor PAF)水平的变化,并探讨其机理。方法:随机将114例急性缺血性脑卒中患者分为降纤酶组和对照组,观察其临床疗效,检测凝血功能和VEGF、ET-1、P-selectin、PAF等4项生物分子水平的变化。结果:治疗后2组临床疗效经Ridit分析差异无统计学意义(P>0.05),2种药物对凝血功能和4项血浆生物分子水平的影响差异有统计学意义(P<0.01),降纤酶抗凝明显,且能降低Fbg水平,对ET-1、P-selectin和PAF有抑制作用,能提高VEGF的血浆含量。低分子肝素钠抗凝明显,也能降低Fbg水平,对PAF有抑制作用。结论:降纤酶可提高血浆VEGF水平,抑制血浆ET-1、P-selectin和PAF的合成。Objective: To explore the changes of plasma vascttlar endothelial growth factor(VEGF), endothelin(ET-1), P-selectin and platelet activating factor( PAF) in patient with ischemic stroke treated with defibrinogenase. Method: The blood coagulation function and the concentration of VEGF, ET-1, P-seleetin and PAF were determined in acute cerebral infarction in 66 patients with defibrinogenase and 48 with low dose heparin before and after treatment. Result: Excluding curative effect, there was significant difference between the two groups on the blood biochemicals items and plasm biomoleculs levels. The Flag decreased, the level of ET-1, P-seleetin and PAF were depressed, and the plasm VEGF level rised in the defibrinogenase group, whereas only Flag and the plasm PAF were depressed by low dose heparin. Conclusion: Defibrinogenase seems to have a salutary effect on descending of the Flag, rising of the plasm VEGF levels and depressing of the synthesis of the ET-1, P-selectin, and PAF.
