完全性唇裂修复术中鼻底裂缘黏膜瓣的应用

摘　　要：目的探讨在完全性唇裂修复术中既能减低鼻底缝合张力,成功封闭鼻底,又简单易行的方法。方法在完全性唇裂患儿鼻底裂隙两侧设计蒂在下及内侧的翻转黏膜瓣,充分松解后,两瓣缘间断缝合闭合鼻底。口轮匝肌及皮肤的缝合,红唇的修复均按常规手术完成。结果用本方法治疗338例完全性唇裂患儿,术后伤口均Ⅰ期愈合,鼻底封闭良好、平整饱满,鼻外形改善满意,伤口瘢痕增生不明显。结论以鼻底裂隙两侧黏膜瓣封闭鼻底,简单有效、张力较低,是完全性唇裂修复术中封闭鼻底的较好方法。Objective To eveluate a technique for not only decreasing the tension of the nasal floor and closing the nasal floor successfully during the procedures but also can being used easily and efficiently by operators to repair the complete clef lip. Methods With the designation of a mucosal flap in each split- ting side of the nasal floor, two mucosal flaps were stitched, the tension was decreased efficiently and the nasal floor was closed easily. 338 patients was selected for the treatment with this technique since 2000. The oral orbicularis, skin and lip were repaired with routine procedures. Results All of the patients showed well wound healing, nasal floor satiation, good nasal shape and light scar hyperplasia. Conclusion The above mentioned technique may decrease the tention of the nasal floor simply and efficiently and close the nasal floor safely. Therefore, it is a better approach for closing the nasal floor in repair of complete clef lip.
