摘 要:目的观察人类微小病毒B19宫内感染(HPVB19)与新生儿疾病之间的关系。方法采用ELISA法,对新生儿贫血、新生儿病理性黄疸、先天性心脏病、其他先天性畸形、足月小于胎龄儿、早产儿共6组173例病理新生儿及48例正常新生儿的血清进行了HPVB19-IgM的检测,结果进行统计学分析。结果(1)HPVB19-IgM阳性率,病理组为29.48%(52/173),正常对照组为4.17%(2/48),二者差异有非常显著性意义(P<0.005),相对危险度(RR)=7.01。(2)HPVB19-IgM阳性率,新生儿贫血组为46.67%(14/30)、新生儿病理性黄疸组为26.67%(8/30)、先天性心脏病组为33.33%(10/30)、其他先天性畸形组为11.11%(2/18)、足月小于胎龄儿组为23.33%(7/30)、早产组为28.57%(10/35),与正常对照组比较(P<0.005,RR=11.2;P<0.05,RR=6.4;P<0.005,RR=8.0;P>0.05,RR=2.67;P<0.05,RR=5.6;P<0.01,RR=6.86)。(3)6组新生儿疾病HPVB19-IgM阳性率及关联强度从高到低依次为新生儿贫血、先天性心脏病、早产儿、新生儿病理性黄疸、足月小于胎龄儿、其他先天性畸形。结论微小病毒B19宫内感染与新生儿贫血、新生儿病理性黄疸、新生儿先天性心脏病、足月小于胎龄儿、早产的发生显著相关。Objective To observe the relation of human parvovirus B19 (HPVB19) intrauterine infection with diseases of newborn infant. Methods The 173 cases of pathological newborn infants were divided into 6 groups - anemia, pathological jaundice, congenital heart diseases, other congenital malformations, full term small for gestational age and premature infant in newborn period.Their level of serum HPVB19 - IgM was measured by ELISA and comared with that of normal newborn group. Result (1) The positive rate of serum HPVB19 IgM was 29.48% (52/173) in pathological groups, 4.17% (2/48) in normal group, there was significantly different in pathological groups and normal group (P 〈 0.005) .Relative risk (RR) was 7.01. (2) The positive rates of serum HPVB19 IgM of anemia group, pathological jaundice group, congenital heart diseases group, other congenital malformations group, full term small for gestational age group and premature infant group were 46.67% (14/30), 26.67% (8/30), 33.33% (10/30), 11.11% (2/18), 23.33% (7/30) and 28.57% (10/35) respectively, and (P〈0.005andRR=ll.2, P〈0.05andRR=6.4, P〈O.005andRR=8.0, P〉0.05 andRR=2.67, P〈0.05 andRR=5.6, and P 〈 0.01 and RR = 6.68) compared with the normal group. (3) The positive late of serum HPVB19 IgM and RR from high to low were anemia, congenital heart diseases, premature infant, pathological jaundice, full term small for gestational age and other congenital malformations in newborn period. Conclusion HPVB19 intrauterine infection is apparently related to the occurrence of anemia, pathological jaundice, congenital heart diseases, full term small for gestational age and premature infant in newborn period.
