耻骨后前列腺癌根治术减少并发症的探讨

摘　　要：目的:探讨耻骨后前列腺癌根治术保留耻骨前列腺韧带及耻骨直肠悬带等对术后并发症的影响。方法:对16例前列腺癌患者(平均年龄67岁,B期14例,C期2例)实施了耻骨后前列腺癌根治术,术中保留耻骨前列腺韧带及耻骨直肠悬带。结果:16例患者手术顺利,随访1～4年,无长期尿失禁及排尿困难,6例保留睾丸者术后4例恢复了性功能,9例患者PSA保持在0.05ng/ml,7例PSA<0.05ng/ml,16例均存活。结论:耻骨后前列腺癌根治术中保留耻骨前列腺韧带及耻骨直肠悬带有助于减少并发症,取得较好的手术效果。Objective：To study the effects of preserving puboprostatic ligament and puborectal sling on postoperative complicastions after radical retropubic prostatectomy.nethods：16 patients with prostate cancer were performed by radical retropubic prostatectomy.The sparing technique of puboprustatic ligament and puborectal sling was used in all patients.Results：All patients recoveved from the operation after follow-up from lto 4 years,none of them had long-term incontinence or dysuria.PSA declined to 0.05 ng/ml and stayed 0.05 ng/ ml in 9 cases,PSA〈0.05 ng/ml in other 7 cases,all 16 patients survived.Conduslon.According to correct anatomic manipulation,preserving puboprustatic ligament and puborectal sling in performing radical retropubic prostatectomy will achieve better outcome.
