脑卒中患者血小板α颗粒膜蛋白的动态监测

摘　　要：目的动态监测脑梗塞和脑出血患者血小板 a 颗粒膜蛋白(GMP-140)的变化。方法用放射免疫法测定脑梗塞和脑出血患者治疗前后血小板 a 颗粒膜蛋白的含量,并与对照组比较。结果治疗前,脑梗塞患者血小板膜表面及血浆内 GMP-140为1979.62±243.65/每个血小板、91.31±29.70μg/L;脑出血患者血小板膜表面及血浆内 GMP-140为1379.67±205.47/每个血小板、85.77±36.30μg/L)均明显高于对照组(780.00±230.D0/每个血小板、46.18±18.23μg/L)(P<0.01)。治疗前脑梗塞患者血小板膜表面 GMP-140分子数高于脑出血者(P<0.01),脑梗塞患者治疗后第2周(1147.52±409.92/每个血小板)与治疗前比较,差异有显著意义(P<0.05),脑出血患者治疗前(1379.67±205.47/每个血小板)及治疗后(1279.44±197.86/每个血小板)差异无显著意义(P>0.05)。脑梗塞患者血浆内 GMP-140浓度在治疗后第一周(63.07±19.90μg/L)明显低于治疗前(P<0.01),脑出血患者在治疗前(85.77±36.30μg/L)及治疗后(58.88±20.67μg/L),差异也有显著性意义(P<0.01)。结论血小板膜表面 GMP-140分子数及血浆内 GMP-140浓度可作为脑卒中的诊断指标,血浆内 GMP-140也是一种比较敏感而特异的疗效探讨。Objective To monitor the variation of the platelet GMP-140 values in cerebral infarc- tion and cerebral haemorrhage patients.Methods Radioimmunoassay was used to measure the platelet GMP-140 levels in cerebral infarction and cerebral haemorrhage patients.Results The cerebral infarc- tion and haemorrhage patients' GMP-140 values on platelet membrane surface(1979.62±243.65/Per- platelet,1379.67±205.47/perplatelet)and in plasma(91.31±29.70/1g/L,85.77±36.30)were sig- nificantly higher than those of tha normal(780.00±230.00perplatelet,46.18±18.23μg/L)(P< 0.01).The platelet GMP-140 molecular numbers on membrane surface before therapy in cerebral in- farction patients were significantly higher than those of the second in week after therapy(1 147.52± 409.92/perplatelet)(P<0.05),and significantly higher than those before therapy in cerebral haemor- rhage patients(P<0.01).The GMP-140 concentrations in plasma of the two-group patients after ther- apy(56.96±22.22/μg/L,58.88±20.67μg/L)were significantly lower than those before therapy. Conclusions The GMP-140 values on platelet membrane surface and in plasma may be a diagnostic in- dicator of cerebral infarction and cerebral haemorrhage diseases.The GMP-140 concentration in plasma is a more sensitive and specific therapeutic indicator than the GMP-140 molecule numbers on the platelet membrane surface.
