摘 要:目的探讨增殖细胞核抗原(PCNA)及nm23、p53和细菌L型感染在卵巢肿瘤中的表达及临床意义。方法应用免疫组化法和革兰染色方法检测101例卵巢肿瘤中的PCNA及nm23、p53、基因的表达及细菌L型的检出率。结果PCNA及p53在恶性肿瘤中的表达率明显高于良性肿瘤(P<0.01);nm23蛋白在恶性肿瘤的表达率则低于良性肿瘤(P<0.01);PCNA及nm23肿瘤转移抑制基因、p53抑癌基因的指标与肿瘤的病理分级及患者预后明显相关(P<0.01~0.05)。细菌L型检出阳性率与病理分级有显著差异(P<0.005),腹腔淋巴结有转移比无转移者、有腹水比无腹水者有显著差异(P<0.01)。结论增殖细胞核抗原(PCNA)及nm23、p53基因在卵巢肿瘤中有不同程度的异常表达,PCNA的表达有助于卵巢良、恶性肿瘤的诊断和鉴别,两种基因均可作为判断卵巢肿瘤生物学行为及患者预后参考指标。L型感染极有可能成为诱发肿瘤因素之一,它们与癌基因可能有协同致瘤作用。Objective To determine the clinical significance of the expressions of proliferative cell nucleo antigen (PCNA ), nm^23 and p53 gene, and the infection with bacterial L- forms in ovarian tumor. Methods Immunohistochemical staining was used to detect the expression of PCNA, nm^23 and p53 gene, and the Grarn's staining to examine the positive rate of bacterial L - forms in 101 cases with ovarian tumor. Results The expression PCNA and p53 was significantly higher in malignant tumors than that in benign tumors (P〈0.01), The positive rate of expression of nm^23 was lower in malignant tumors than that in benign tumors (P 〈 0.05), PCNA and nm^23 tumor metastasis suppressor genes and p53 anti oncogenes have significant relationship with the pathological grades and the prognosis for patients (P 〈0.01 -0.05). There was obvious difference (P 〈 0. 005) between the possitive rate of bacterial L- forms and pathological grades. The positive rate in patients with celiac lymph node metastasis and ascites was significant higher than in that without the metastasis and ascites ( P 〈 0.01 ). Conclusions PCNA, nm^23 and p53 have certain positive expression in ovarian neoplasms. The positive expression of PCNA are helpful to the diagnosis and differentiation of malignant and benign tumors, and both of the two genes can be used as appraise markers of the biological behavior of tumors and the prognosis for patients. Infection of bacterial L - forms is probably one of the inductive factors of the oncogenesis, and has positive cooperativity with the oncogene during the course.
