摘  要:目的探讨一氧化氮(NO)、内皮素(ET)对肝硬化患者动脉氧分压(PaO2)的影响。方法用硝酸还原酶法检测51例肝硬化组(其中Child-PughA、B、C级分别为19、21、11例)和31例对照组血浆中NO水平;放射免疫法检测受试对象血浆中ET水平;分别用自动血气分析仪检测动脉血氧水平和自动生化分析仪检测受试对象肝功能状态。结果肝硬化A、B、C三组动脉血氧分压(PaO2)显著低于对照组(31例)[分别为(79.43±6.33)mmHg(、78.88±8.99)mmHg及(72.60±5.27)mmHg对(88.33±6.60)mmHg(P<0.01)];肝硬化低氧血症组血浆中NO、ET水平及NO/ET值较非低氧血症组均明显升高。结论肝硬化患者动脉血氧分压与血浆中NO和ET水平及NO/ET值有明显负相关性,血浆中NO、ET及NO/ET的失衡在肝性低氧血症发病机制中有着重要作用。Objective To investigate the influence of nitric oxide(NO) and endothelin(ET)levels on the arterial partial pressure of oxygen (PO2) in patients with liver cirrhosis, Methods The plasma levels of NO and ET of 51 cirrhotic patients (liver function Child - Pugh grading: grade A, 19 eases; grade B, 21 cases; grade C 11 eases ) and 31 normal individuals ( control ) were measured by Griess and radioimmunoassay respectively;The arterial oxygen level and liver function were measured by automatic blood gas analyzer and automatic biochemistry analyzer. Results (1)The arterial oxygen level in cirrhotic patients [A: (79.43±6.33)mmHg; B: (78.88± 8.99)nmfflg C: (72.60 ±5.27)mmHg] was significantly lower than that in control group [88.33 ±6.60 mmHg] respectively ( P〈0.01 ) ; (2) The plasma levels of NO and ET and the ratio of NO/ET were significantly higher in cirrhotic patients with hypoxemia than that with no hypoxemia (P〈0.01). Conclusions Arterial oxygen level had a negative correlation with the plasma levels of NO and ET, and the ratio of NO/ET. The disproportion of NO and ET plays an important role in pathogenesis and development of hepatic hypoxemia of cirrhotic patients,
