摘 要:目的探讨粪中脱落细胞端粒酶活性检测筛查大肠癌的敏感性和特异性。方法对45例大肠癌患者,34例非大肠癌患者,从新鲜大便中分离脱落细胞并裂角,用聚合酶链端粒重复扩增(PCR-TRAP)银染技术观察端粒酶活性表达情况。同时采用试纸法做受检者粪隐血试验。结果45例大肠癌患者粪便标本中有30例端粒酶阳性表达,1例大肠腺瘤、1例溃疡性结肠炎端粒酶表达为阳性,提示粪便脱落细胞端粒酶活性检测对大肠癌的敏感性为66.67%,特异性为94.12%。粪便隐血试验中大肠癌的敏感性为82.22%,特异性为58.82%。两种方法对大肠癌阳性检出率差异无显著性(P>0.05)。端粒酶阳性表达与其大肠癌Duke’s分期、淋巴结转移和癌肿部位均未见显著相关(P>0.05)。结论粪中脱落细胞端粒酶活性检测有望成为一种新的大肠癌筛选方法。Objective To probe into the sernsitivity and specificity of screening colorectal cancer by the telomerase activity in exfoliated cells of stool, Methods 45 cases with colorectal cancer and 34 cases without ooloreetal cancer were studied. The exfoliated cells were isolated from fresh stool and cracked by Lysis reagent. The telomerase activity was determined by PCR - TRAP, While the fecal occult blood test (FOBT) was done in all cases. Results The telomerase activity of exfoliated cells in stool is positive in 30 cases with colorectal cancer, 1 case with colorectal adenomas, and 1 case with chronic colitis. The sensitivity and specificity of the test of telomerase activity in the exfoliated cells from the stool was 66.67% and 94.12% respectively, while FOBT was 82.22% and 58.82% respectively for the colorectal cancer. There was no significant difference in the .sen sitivity between the two methods( P 〉0.05 ). Telomerase activity was not obviously correlated with Duck's stage, lymphatic metastasis and tumor location (P〉0.05), Conclusions The determination of telomerase activity in the exfoliated calls isolated from stool will be a new method in the screening of colorectal cancer.
