IGF-1增加脓毒症大鼠骨骼肌葡萄糖载体及其表达

摘　　要：目的:探讨胰岛素样生长因子-1(IGF-1)对脓毒症大鼠血浆应激激素及骨骼肌葡萄糖载体-4(GLUT-4)和mRNA表达的影响。方法:将SD大鼠采用盲肠结扎穿孔法(CLP)建立腹腔感染及颈静脉置管肠外营养(PN)模型后,随机分为脓毒症组(n=10)、PN组(n=10)和IGF-1组(n=10),另设正常组(n=10)仅行颈静脉置管输液。正常组和脓毒症组给予等渗盐水,自由进食和饮水,PN组给予营养支持,IGF-1组为PN+IGF-1静脉注射,连续5天。第6天测定血糖、胰岛素、高血糖素和IGF-1水平;免疫组化法测定骨骼肌GLUT-4及Real-Tim e PCR法测定GLUT-4 mRNA表达。结果:脓毒症大鼠血浆葡萄糖、胰岛素、高血糖素明显升高,IGF-1明显下降(P<0.01)。IGF-1组前三者较脓毒症组和PN组明显降低,IGF-1则明显升高(P<0.05)。脓毒症组骨骼肌GLUT-4及mRNA表达明显降低(P<0.01);IGF-1组GLUT-4及mRNA表达较脓毒症组和PN组明显增加(P<0.01)。结论:IGF-1可改善应激激素之间的平衡,明显增加脓毒血症大鼠骨骼肌GLUT-4蛋白和mRNA的表达。Objective： To investigate the influences of IGF-1 on the stress hormone, GLUT-4 and its mRNA expression. Methods： The SD rats with abdominal infection established by cecal ligation and perforation method were randomly divided into three groups： pyaemic group （n = 10）, parenteral nutrition group（PN group,n = 10） and IGF-1 Group （n = 10）. 10 healthy rats was used as the normal group （n = 10）. On the sixth day, blood was sampled to determine the level of glucose, insulin, glucagon and IGF-1. The expressions of GLUT-4 and its mRNA in muscle were detected by immuno- histochemistry and Real-Time PCR methods. Results ： The plasma levels of glucose, insulin and glucagon in the IGF-1 group were significantly lower than those in the pyaemic group and PN group （ P 〈 0, 05 ）, The muscle GLUT-4 and expressions of GLUT-4 mRNA in the pyaemic group and PN group were lower than Those in of the normal group. The expressions of GLUT-4 mRNA in the tGF-1 group were significantly higher than those in the of pyaemic group and the PN group （ P 〈 0.01 ）. Conclusion ： IGF-1 can significantly improve the balance of the stress hormone and increase the GLUT-4 and the expression of GLUT-4 mRNA.
