中心性浆液性脉络膜视网膜病变早期氪激光治疗疗效评价

摘　　要：目的评价中心性浆液性脉络膜视网膜病变(cen-tral serous chorioretinopathy,CSC)早期激光治疗的效果。方法回顾性分析44例(44眼)CSC初发早期(病程<3月)病例的激光光凝治疗疗效和随访结果,并与同期CSC初发晚期(病程≥3月,13例13眼)和复发病例(11例11眼)的相应治疗比较。结果初发早期CSC病变早期激光治疗,在治疗后1月和3月内,可分别获得77·3%和90.9%显效率,明显优于同期CSC初发晚期和复发病例的相应疗效(P<0.05);激光治疗后6月、12月和36月的随访期内CSC病情迁延不愈或反复发作的比率分别是2.4%、4.8%和10.0%,也明显少于其他组别的相应比率(P<0.05)。结论氪激光治疗CSC可较快提高视力、缩短病程、减少复发率。病程<3月的CSC初发早期病例是激光治疗的最佳、需(极力争取)及时治疗的指征。Objective To evaluate the therapeutic effect of Krypton laser photocoagulation on central serous chorioretinopathy （CSC） in its early stage. Methods Data of 68 patients with CSC（68 eyes） undergone Krypton laser therapy were analyze retrospectively, and the comparison was performed between the therapy outcomes, the post-therapy recurrence in their CSC early stage （less than 3 months from its initial onset, 44 cases, 44 eyes）, non-early stage（ 3 months from its initial onset, 13 cases, 13 eyes） or recurrent stage （ 11 cases, 11 eyes）. Results The success rate of Krypton laser therapy for patients in their early CSC stage was 77.3% when assessed at the follow-up after one month of the therapy and 90.9% at the three-month follow-up, which is significantly higher than tho6e patients in their CSC non-early stage （ P 〈 0.05 ） or recurrent stage （ P 〈 0,05）. The post, therapy protraction or recurrent rate of CSC in patients who received the Krypton laser therapy in their CSC early stage was 2.4% ,4.8% and 10.0% respectively when assessed at the follow-up after 6 months, 12 months and 36 months of the therapy, which was significantly lower than tho6e patients in their CSC non-early stage or recurrent stage （P 〈 0.05）. Conclusion In order to aquire a rapid amelio- ration in eyesight, a short treatment duration and a low recurrence, it is reasonable and recommendable for patients in their early CSC stage to receive the Krypton laser therapy.
