口腔扁平苔藓病损区角朊细胞表达HLA－DR抗原的意义

摘　　要：为进一步探讨口腔扁平苔藓（ｏｒａｌｌｉｃｈｅｎｐｌａｎｕｓ，ＯＬＰ）的发病机制，作者用免疫组织化学ＰＡＰ法检测了ＯＬＰ病损区角朊细胞（ｋｅｒａｔｉｎｏｃｙｔｅ，ＫＣ）异常表达人白细胞分化抗原ＤＲ区（ｈｕｍａｎｌｅｕｃｏｃｙｔｅａｎｔｉｇｅｎ－ＤＲ，ＨＬＡ－ＤＲ）抗原的情况。结果显示，所有标本的粘膜上皮中均有ＨＬＡ－ＤＲ阳性ＫＣ，其数量和表达程度与ＯＬＰ的病程及类型有关，不同组间差异有高度显著性（Ｐ＜０．０１）。作者认为，ＯＬＰ病损区ＫＣ异常表达ＨＬＡ－ＤＲ抗原对Ｔ淋巴细胞在局部的聚集、炎性病灶的形成是至关重要的。Inthisarticle,theexpressionofHLA-DRinkeratinocytesinlesionsoforallichenplanus(OLP)wasinvestigated,usinganimmunohistochemicaltechnique(PAPmethod).There-sultsshowed:HLA-DRpositivekeratinocyteswerefoundinalgroupsofpatients,thenumbersandexpressivedegreeofHLA-DRwererelatedtotypesanddurationofOLP,therewerehigherstatisti-calysigni-ficantdiferencesbetweendiferentgroupsandtypesofOLP(P<0.01).TheauthorsconsideredthattheexpressionofHLA-DRinkeratinocytesplaysanimportantroleingatheringTcelsandforminginflammationinOLPlesions.
