奥美拉唑对胃癌前病变细胞凋亡的影响

摘　　要：目的:研究奥美拉唑治疗对胃癌前病变细胞凋亡的影响。方法:胃镜活检经病理证实为胃癌前病变患者32例,随机分为治疗组(奥美拉唑20mg,qd)与对照组(硫糖铝1.0g,tid)各16例,分别利用逆转录聚合酶链式反应(RT-PCR)和TUNEL方法检测治疗前、后胃癌前病变组织bcl-2、baxmRNA表达率以及凋亡指数的变化。结果:奥美拉唑组治疗后细胞凋亡指数和baxmRNA表达率明显升高,而bcl-2mRNA表达率降低;对照组治疗前后各项指标均无明显变化。结论:奥美拉唑可诱导胃癌前病变组织细胞凋亡,其机制可能与增强baxmRNA的表达,抑制bcl-2mRNA的表达有关。OBJECTIVE To study the effects of omeprazole on inducing gastric epithelium apoptosis in precancerous condition of gastric carcinoma. METHODS Gastric biopsy specimens were taken by endoscopy from 32 patients with precancerous condition. The patients were randomly divided into treatment group（n = 16,omeprazole 20 mg, qd） and control group （n = 16, sucralfate 1. 0g, tid）. The mRNA expressions of bcl-2 gene and bax gene were determined by RT-PCR and inducing gastric epithelium apoptosis were determined by TUNEL. After therapy the mRNA expressions of bcl-2, bax and gastric epithelium apoptosis were reexamined. RESULTS After therapy of omeprazole gastric epithelium apoptosis and the expression of bax gene was increased,while the mRNA expression of bcl-2 gene was decreased. CONCLUSION Omeprazole could induce gastric epithelium apoptosis by increasing bax gene expression and inhibiting bcl-2 gene mRNA expression.
