带肌蒂锁骨复合瓣移植修复下颌骨缺损

摘　　要：报告以胸锁乳突肌为蒂的锁骨复合的即刻重建下颌骨缺损１３例，并作了长期追踪（１１月～７年）。除首例因术后感染至部分移植的锁骨外端吸收外，余１２例均获成功（其中６例还同时携带该肌肌皮瓣修复了相应部位的软组织缺损，亦全部成活）。锁骨瓣自带可靠血供，移植后仍保持活力，并参与骨修复，具有愈合迅速、抗感染力强、不畏吸收，基本保持原有形态和体积，因而术后可获得满意的外形和功能。若伴有口腔衬里或面部皮肤缺损时，还可携带该肌肌皮瓣，同期进行修复，因此，本法在口腔颌面部缺损的修复中，是一种较理想值得推荐的方法。Reconstruction of mandibular defect with a pedicled sternocleidomastoid muscle-clavicle flap was done on 13 cases with a follow-up observation of 11 months to 7 years.All were successful except the first patient whose transplanted clavicle was absorbed owing to pest-operative infection.The loss of soft tissues in 6 patients were also repaired with success.The grafted flap had its own reliable blood supply that resulted in good restoration both functionally and cosmetically.Based on the advantages such as quick wound healing,infection resistance and virtue of anti-absorption it afforded satisfaction to the patients.
