区域热·药灌注治疗肢体恶性肿瘤的并发症及其预防（附52例报告）

摘　　要：５２例使用区域热·药（ＤＤＰ３ｍｇ／ｋｇ）灌注治疗的肢体恶性肿瘤患者，其中４５例并发恶心呕吐（Ⅰ－Ⅱ级）和患肢疼痛；４０例出现受灌注肢体肿胀；３例发生间室综合征，无肾功能受损，骨髓抑制和贫血等。治疗与预防方面强调组织间温度严格掌握在４１．５℃左右，不能超过４３℃以及局部暂时完全阻断的重要性。交感神经节切除能预防发生肢体肿痛，区域热·药灌注如掌握得当不失为安全有效的治疗方法。The complications of hyperthermic regional perfusion in the treatment of 52 patients with malignant tumors of the extremities are reported. Nausea,vomiting and local gain were common occurrences in 45 cases,swelling of extremity in 40,and compartment syndrome in 3.Complications such as renal impairment,bone marrow suppression or anemia did not occur.It is emphasized that the tissue temperature of the perfused limb should be maintained at 41. 5℃ and not beyond 43℃.It is also very important to keep the perfusing region isolated from the systemic circulation as complete as possible.Sympathectomy can be effective in preventing pain and swelling of the extremity from occurrence.It is concluded that regional perfusion with hyperthermia and drug is a safe and effective method in the treatment of malignant tumors of the extremities if aPPlied judiciously.
