鼻咽癌放疗结果与预后相关因素

摘　　要：１９７６～１９９２年首程根治放射治疗鼻咽癌３１９例。１、３、５、１０年存活率分别为８０．９％、６４．１％、５８．１％、３０．５％。Ⅰ、Ⅱ期病人生存质量及生存率明显高于Ⅲ、Ⅳ期。影响预后的主要因素为病期及放射剂量，病期早生存率高。鼻咽癌放射剂量应在６０～７０Ｇｙ６～７周为宜。其它因素如年龄，病理类型，临床表现与生存率无关。建议鼻咽癌耳前野后界向后移动０．５～１ｃｍ，使后界垂直外耳孔或外耳孔后缘，避免咽后壁、咽旁间隙肿瘤复发。From 1976 to 1992,319 cases of NPC were treated by curative radiotherapy at authors' hospital.The 1-, 3-, 5- and 10-year survival rates were80. 9%,64. 1%,58.1% and 30.5% respectively.The quality of life and the survivalrate for patients with stage Ⅰ and stage Ⅱ diseases were better than those withstage Ⅲ and stage Ⅳ.Factors affecting the survival were staging and dosage.Theradiation dose rangedf orm 60-70Gy/6-7w.The authors suggest that the Posterior margin of the preauricular portal shouldmove backward by 0.5-1cm and rest on the external auditory meatus or just behindit to avoid recurrence in the parapharyngeal space.
