液基细胞薄片以及阴道镜在宫颈癌及宫颈上皮内瘤变筛查中的应用

摘　　要：目的：探讨液基细胞薄片制备（TCT）联合阴道镜在宫颈上皮内瘤变（CIN）及宫颈癌筛查中的应用价值。方法：以我院2004年6月-2005年1月妇科门诊800例患者为研究对象，行液基细胞薄片制备，采用TBS分级诊断系统，同时进行阴道镜下宫颈多点活检，以组织学检查为金标准，分析TCT与阴道镜检查在宫颈癌前病变诊断中的应用价值。结果：病理组织学检查发现CINⅠ级135例，CINⅡ级26例，CINⅢ级14例，宫颈癌8例，其余均为良性细胞改变。TCT细胞学诊断检测到CINⅠ级以上病变与组织学检查相比，阳性符合率86．89％，查到癌细胞的5例全部相符，细胞学检查阳性而活检结果为炎症者18例，占9．84％。阴道镜检查结果提示CINⅠ级以上病变与组织学检查相比，阳性符合率80．62％。结论：TCT结合阴道镜检查用于宫颈上皮内瘤样病变及宫颈癌筛查，敏感性高，假阴性率低，是一种高效、便捷的方法，有重要的应用价值。Objective：To study the utility of ThinPrep cell test （TCT） and eolposeope in screening of cervical carcinoma and cervical intraepithelial neoplasia. Methods： TCT were performed in 800 patients in our department from June 2005 to January 2006 and the Bethesda System （TBS） was used for diagnosis. All these patients were examined by biopsies at multiple sites under the guide of eolposcope. Results： Among 800 patients investigated, 135 patients were diagnosed with CINⅠ, 26 patients with CINⅡ, 14 patients with CINⅢ and 8 patients with cervical carcinoma by histopathology. 25 patients were diagnosed with squamous cell of undetermined significance （ASC） and 121 patients were found to be low - grade squamous intraepithelial lesion （LSIL）, 33 patients to be high - grade squamous intraepithelial lesion （HSIL） and 5 cases with cervical carcinoma were detected by TCT. 130 cases with CINⅠ, 19 ones with CINⅡ, 8 ones with CINⅢ and 9 ones with carcinoma were detected by eolposeope. In the comparison with the results of CIN and cervical carcinoma by biopsy, the agreement rate of TCT and eolposeope was 86. 89% and 80. 62%, respectively. Conclusion： It is demonstrated that TCT and colposcope play important role in screening of CIN and cervical cancer with high sensitivity and specificity.
