交感性眼炎远期疗效分析

摘　　要：目的：评价以中药为主的中西医结合方法治疗交感性眼炎的远期疗效。方法：２６例本病患者随访１～７２年，平均８．７３年。局部均用西药治疗，１８例早期用激素后，因病情反复或有副作用，其中１６例改为中西药结合疗法；８例早期仅接受中药治疗。评价全组疗效，并比较与早期应用激素及中药的差异。结果：全组最后视力≥１．０者８０．７７％，≥０．６者８８．４５％，≥０．１者９２．３％；病程迁延＞１年者为２３．０８％；复发率为２６．９２％；继发青光眼及或并发白内障发生率为１９．２３％。早期用中药组比用激素者疗效佳。一般前者无复发，无继发青光眼及或并发白内障，无全身并发症，３个月疗程可治愈，视力恢复好。结论：中西医结合治疗交感性眼炎安全有效。为预防交感性眼炎，建议于眼球穿通伤后，如伤眼尚可挽救，立即服用中药２～３个月。OBJECTIVE. Evaluate the long-term therapeutic effect of integrated treatment oftraditioiial and western medicine on sympathetic ophthalmic(SO).METHOD:Twenty six cases of SO were followed for 1-22(mean 8.73) years.All cases were treated With western medicine locally.18 cases were treated With systemic steroid at early stage, but relapses and,or side effects occurred。Thus 16 cases were switched to the integrated treatment of tradit ional and western medicine 8cases received systemic only TCM at early stage. The therapeutic effects of alI the cases were ana-lyzed. The effect in the group with early administration of systemic steroid and the group withTCM were compared statistically.RESULTS: The cases with final visual acuity(FVA)>1.0account for 80.77%;≥O6,88.45%;≥0.1,92.3%. The rale of the disease course long-lasting over 1year is 23.08%。 The rate of recurrence. 26.92%. The rate of development of secondaryglaucoma(SG)and. or complicated cataract(CC)19.23 %. In comparison to those in the groupwith early steroid, the therapeutic effects in the group with early TCM are much better:In thegroup with early systemic TCM treatment, generally,there is no recurrence,no SG and or CCand no side effects. and the disease can be cured within 3 months with good visual recovery,CONCLUSION: The integrated therapy is safe and effective for treatment of SO.To prevent onset of SO, it is suggested to take TCM for 2-3months immediately after penetrating ocular trauma in a savable eye.
