温下法与温涩法对溃疡性结肠炎外周血T淋巴细胞亚群的影响

摘　　要：[目的]探讨温下法与温涩法在溃疡性结肠炎(UC)细胞免疫调节方面的不同作用机制。[方法]采用流式细胞仪三色标记法检测治疗前后温下组、温涩组和对照组的外周血T淋巴细胞亚群的水平,比较3组治疗前后的变化。[结果]治疗前,温下组、温涩组和对照组外周血CD8+细胞绝对计数均显著高于正常参考值(P<0.05),CD4+/CD8+比值均显著低于正常参考值(P<0.05);温下组与对照组外周血CD3+、CD4+细胞绝对计数亦显著高于正常参考值(P<0.05)。治疗后,温下组外周血CD3+、CD4+与CD8+细胞绝对计数变化差值均显著高于温涩组与对照组(P<0.01);温涩组外周血CD3+与CD8+细胞绝对计数变化差值均显著高于对照组(P<0.01),CD4+/CD8+比值变化差值高于温下组与对照组(P<0.05)。[结论]①UC患者外周血T淋巴细胞亚群的变化总体以CD8+细胞水平升高及CD4+/CD8+比值下降为主,而CD4+细胞绝对数增多与其活动性有关。②温涩法不但能显著降低CD3+细胞计数和CD8+细胞计数,还能提高CD4+/CD8+比值;温下法能显著降低CD3+、CD4+与CD8+细胞计数,但不能提高CD4+/CD8+比值。[Objective] To study the different mechanism of warm and purgative remedy （WPR） and warm and restraining diarrhea remedy （WRDR） on cytological immunological adjustment in ulcerative colitis （UC）. [Methods] Expression level of the T-cell subsets from the three groups of WPR, WRDR and the control was investigated by 3-color immunofluorescence flow-cytometry （FCM） analysis in fresh peripheral blood lymphocytes （PBL） before and after the respective treatment. [Results] Before treatment, the absolute number of CD8^＋ cells of WPR, WRDR and the control group was obviously higher than that of the normal referenced numerical value respectively （P〈0. 05）, the ratio of CD4^＋/CD8^＋ of the three groups was obviously lower than that of the normal referenced numerical value（P〈0. 05）, and the absolute number of CD3^＋ and CD4^＋ cells of WPR group and the control group was obviously higher than that of the normal referenced numerical value respectively （P〈0. 05）. After treatment, the discrepant numerical value of absolute number of CD3^＋ , CD4^＋ and CD8^＋ cells of WPR group was significantly higher than that of WRDR group and the control group（P〈0. 01）, the discrepant numerical value of absolute numbers of CD3^＋ and CD8^＋ cells of WRDR group was significantly higher than that of the control group（P〈0. 01）, and the discrepant numerical value of the ratio of CD4^＋/CD8^＋of WRDR group was significantly higher than that of WPR group and the control group（P〈0. 05）. [Conclusion] （1）The specific character of T-cell subsets in UC patients chiefly is the increase of CD8＋ cells level and the decrease of CD4^＋/CD8^＋ ratio, and the increase of the absolute numbers of CD4^＋ cells is in related to its activity. （2）The WRDR can not only evidently decrease the absolute numbers of CD3^＋ and CD8^＋ cells , but also increase the ratio of CD4^＋/CD8^＋.
