肺硬化性血管瘤的诊断和治疗——附27例报告

摘　　要：回顾性分析天津医科大学附属肿瘤医院1997年1月～2005年1月收治的肺硬化性血管瘤27例患者的临床资料,对其临床表现、实验室检查、治疗方式、病理特点和预后等资料进行了分析。结果示肺硬化性血管瘤占同期肺切除术的1.05%(27/2560)。全部27例均有随访资料,随访时间3个月～7年。复查X线胸片,均未见复发。初步研究结果提示,PSH是一种可能以上皮细胞为主要来源的真性肿瘤,可以出现复发和转移,女性多见,临床上多无典型的症状体征,X线及CT等检查多无明显特征,主要治疗以手术切除为主,且应连同周围肿大的淋巴结一并切除,预后良好,极少见转移。Pulmonary sclerosing hemangioma is a kind of rare neoplasm of lung, but it has been taken for a nontumorous disease in the past. This paper was written to analyze and discuss its clinical manifestations, treating method and tissue origin etc. This paper retrospectively analyzed 27 examples of PSH in all which had been adopted in Tianjin Medical University Tumor Hospital from January 1997 to January 2005 and occupied 1.05% （27/2 560） of lung surgical resection in the same period. All the 27 examples had been given operations, so their clinical manifestations, laboratory examinations, treating methods, pathological characteristics and prognoses have been analyzed. All the pationts had been released from hospital after recovery. All of them have follow-up materials, and the follow-up time ranges from 3 months to 7 years. No recurrence happened according to lung X-rays of reexamination. In conclusion, PSH is a kind of true neoplasm whose main source may be epithelial cell and which may recur and metastasize. It mostly happens to women and has no typical clinical symptoms and pathological signs. No obvious characteristics have been shown in the examinations through X-rays and CTs etc. The main method for treating PSH is the resection by operation and PSH should be removed together with the adjacent lymph nodes. The prognosis is fine and no recurrence or metastasis happens.
