摘 要:目的探讨在长期饮酒的基础上摄入不同量的蛋白质对胰腺腺泡细胞结构和功能的影响。方法Wistar大鼠分为酒精组(酒精+常规蛋白质组、酒精+高蛋白质组、酒精+低蛋白质组)和对照组,观察6个月。在光镜和电镜下观察胰腺腺泡细胞结构变化,用比色法检测胰腺组织匀浆淀粉酶和脂肪酶的含量,TUNEL法检测腺泡细胞凋亡,免疫组化检测胰腺组织切片中COX-2的变化。结果与对照组相比,酒精组大鼠胰腺腺泡细胞可见髓样结构形成。酒精+常规蛋白质组腺泡细胞损伤最轻,酒精+高蛋白质组胰腺腺泡细胞内线粒体普遍肿胀,酒精+低蛋白质组主要表现为粗面内质网明显扩张,甚至部分细胞胞质溶解坏死。对照组淀粉酶含量为(6532.87±100.11)U/g组织、脂肪酶含量为(10193.06±206.27)U/g组织,与对照组比较,各酒精组淀粉酶及脂肪酶含量均有不同程度的降低(P<0.01)。对照组胰腺腺泡细胞凋亡指数为(0.31±0.10)%,酒精+低蛋白质组胰腺腺泡细胞凋亡指数明显增加(P<0.01)。对照组COX-2的ILD为0.09±0.02,酒精+低蛋白质组胰腺腺泡细胞胞质COX-2的ILD为0.22±0.03,表达明显增加(P<0.01)。结论长期酒精摄入导致胰腺腺泡细胞退行性变,饮食中适量的蛋白质摄入对延缓胰腺病变发展十分重要。过高或过低蛋白质摄入都将促进腺泡细胞结构的破坏,加速酒精性胰腺损伤的进程。Objective To investigate the effects of dietary protein modification on morphology and function of pancreatic acinar cells in rats on long-term alcohol consumption. Methods Wistar rats were fed with diet containing ethanol of 25 % concentration and protein of different concentrations for 6 months. Histological studies were done by light microscopy and electron microscopy. Lipase and amylase were assayed in pancreatic tissue homogenate by colorimetry. Apoptosis and expression of cyclooxygenase-2(COX-2) in acinar cell were detected by TdT-mediated dUTP nick end-labeling assay (TUNEL) and immunohistochemical staining, respectively. Results Compared with control group, myelin figure was observed in ethanol-fed group. Electron microscopic evaluation revealed enlarged mitochondria in high-protein plus alcohol diet group, and dilation of endoplasmic reticulum in lowprotein plus alcohol diet group. Lipase and amylase were (6532.87 ± 100.11)U/g tissue and (10193.06 ± 206.27)U/g tissue in control group, respectively. Compared with control group, lipase and amylase were significantly decreased in ethanol-fed group (P 〈 0.05). The expression of COX-2 and apoptosis indices were significantly increased in low-protein plus alcohol diet groups (P 〈 0. 05). Conclusions Long-term alcohol consumption induces acinar cell retrogression and lowers pancreatic exocrine function. Moderate diet protein intake is significant in slowing down the progress of pancreatic acinar cell retrogression.
