釉基质蛋白治疗牙周骨下袋缺损促进形成牙周硬组织的能力:数字化标准系列放射学观察

摘　　要：目的:观测分析釉基质蛋白治疗牙周骨下袋缺损前后拍摄的数字化标准系列X线片,以评估釉基质蛋白促进形成牙周硬组织的能力。方法:选择2000-01/2002-01上海第二医科大学附属第九人民医院口腔内科收治的无全身系统性疾病的慢性牙周炎患者24例,共40个牙位。按照牙位随机分为3组,釉基质蛋白组17个牙位,空白对照组13个牙位,甲壳素膜组10个牙位。各组患者在患牙区行改良Widman翻瓣术,釉基质蛋白组骨缺损区根面放置含釉基质蛋白的甲壳素膜,甲壳素膜组放置甲壳素膜,空白对照组不放任何材料仅做术区清创。各组分别于术前和术后6个月测定数字化X影像指标:釉牙骨质界到牙槽嵴顶的距离、釉牙骨质界到骨缺损底部的距离、牙槽嵴顶到骨缺损底部的距离。所有测量值的单位均由Cygnus系统的默认值换算为mm。结果:实验纳入40个牙位,全部进入结果分析。各组各项指标治疗前与治疗后6个月差值的比较:与釉基质蛋白组比较,甲壳素膜组和空白对照组治疗前后釉牙骨质界到牙槽嵴顶的距离差值均有所升高[(0.13±0.28),(0.20±0.41),(0.20±0.35)mm],釉牙骨质界到骨缺损底部的距离、牙槽嵴顶到骨缺损底部的距离差值均明显下降[(1.40±1.16),(0.35±0.56),(0.61±0.72)mm,P<0.05;(1.53±1.17),(0.55±0.79),(0.81±0.76)mm;P<0.01]。结论:釉基质蛋白治疗能够使骨缺损区的骨质修复量及骨缺损深度明显减少,可显著促进牙周骨组织的再生,充分显示了釉基质蛋白治疗牙周骨下袋缺损的优势。AIM： To observe and analyze the digitalization-standarized X-ray taken before and after intrabony defects of periodontal bone with enamel matrix proteins （EMPs）, so as to evaluate the ability of EMPs promoting the regeneration of periodontal tissues. METHODS： We chose 24 chronic periodontitis without systemic disease who received the treatment in the Department of Oral Medicine, the Ninth People＇s Hospital Affiliated to Shanghai Second Medical University from January 2000 to January 2002. There were totally 40 intrabony defects. Intrabony defects were randomly divided into 3 groups： EMPs group （n=17）, blank control group （n=13） and chitin membrance group （n=10） . All the intrabony defects were given the treatment with flap surgery plus chitin membrane absorbed combined with EMPs, with flap surgery plus chitin membrane, and with flap surgery only in the three group above-mentioned respectively . Digital X-ray indexes of each group were determined before and 6 months after operation： distance from the enamelo-cemental junction to the alveolar crest , distance from enamelo-cemental junction to base of the defect and the distance from the alveolar crest to the base of the defect. The unit of all the data measured were conversed from default value of Cygnus system into mm. RESULTS： Totally 40 intrabony defects were enrollled in this experiment and all of them entered the stage of result analysis. Comparasion of the difference of each index before and 6 months after operation in each group： the difference of distance from the enamelo-cemental junction to the alveolar crest before and after operation was significantly increased in the chitin membrance group and blank control group than in the EMPs group [（0.13±0.28）, （0.20±0.41）, （0.20±0.35） mm]; the difference of distance from enamelo-cemental junction to base of the defect and the distance from the alveolar crest to the base of the defect was significantly decreased in the chitin membrance group and blank control group
