奥铂治疗恶性心包积液的临床疗效观察

摘　　要：目的 观察奥铂治疗恶性心包积液的临床疗效。方法 本组29例，支气管肺癌21例（NSCLC19例，SCLC2例），乳腺癌4例，食管-胃癌4例。随机分组：奥铂治疗组15例，对照组（顺铂）14例。超声引导下心包置管引流，心包腔注入奥铂（治疗组）50～100mg／次或顺铂（对照组）30～50mg／次，5％GS或NS稀释至20～40ml，间歇性给药2～3次。结果 奥铂治疗组和对照组的完全缓解率及有效率分别为26．7％、21．4％和73．3％、64．3％（P〉0，05）。奥铂治疗组与对照组的疗效差异无显著性，毒副作用轻微，无心脏骤停，心肌、冠状血管损伤等严重并发症。结论 奥铂治疗恶性心包积液疗效显著，毒副作用轻。Objeetive To study the effect of Oxaliplatin （L-OHP） in the treatment of Malignant Hydropericardium. Methods 29 patients with malignant hydropericardium were lung cancer 21 cases（NSCLC 19 cases, SCLC 2 cases） , breast cancer 14 cases and esophagogastric cancer 4 cases, were divided into 2 groups by random. One group of 15 patients with L-OHP and the other of 14 patients was treated with DDP. L-OHP （50～100mg） or DDP（30～50mg） ,diluted to 20～40ml with 5%GS or NS, were injected into pericardium and may given tow to three times at regular interval after catheter was set into pericardium by B Ultrasound-guided and drained hydropericardium. Results The complete remission and response rates of L-OHP and DDP groups were 26. 7%, 21.4% and 73. 3%, 64. 3%（P〉0. 05）. There was not significant different statistically between L-OHP and DDP groups in therapeutic effect. There were light in toxic reactions and no cardiac arrest, damage of myocardium and coronary vascule, and no catheter fracture, infection and obstruction. Conclusion There is significant therapeutic effect of L-OHP in the treatment of malignant hydropericardium.
