摘  要:目的观察吸附有骨形态发生蛋白(BMP)及碱性成纤维细胞生长因子(bFGF)的异体脱抗原松质骨(AACB)植入股骨头坏死(FHN)病灶清除区后的病理变化过程,评价bFGF对FHN的再血管化作用。方法建立液氮冷冻诱导性犬FHN缺损坏死模型,以该种材料充填骨腔。术后行股骨头组织学大体观察、组织切片免疫组化血管染色并进行血管计数和血管面积图像分析。结果组织学大体观察,3周时大量新生组织长入移植物内;6周时大量新生骨小梁长入;12周时骨小梁已布满充填区。血管计数,术后3周移植孔隙内大量血管增生;6周、12周时血管数量进一步增加。血管面积,术后3周血管截面积占视野的18.27%;6周、12周时血管截面积进一步加大。结论AACB是生长因子的良好吸附载体,适宜新生血管长入。吸附有bFGF及BMP的AACB具有较强的再血管化能力,能有效地提高骨坏死病理条件下股骨头的骨修复能力。这一疗法有望成为FHN治疗的一种手段。[Objectives] To observe the pathological changes after BMP/bFGF-adsorbing AACB (allogeneic antlgen-extracted cancellous bone) being infdled into the necrotic areas of the femoral head, and evaluate the revascularizhtion effect of bFGF on the femoral head necrosis(FHN). [Methods] BMP/bFGF-adsorbing AACB was infilled into the defective and necrotic areas of the femoral heads in canines after the defect had been frozen by liquid nitrogen. Gross histology, immuhitochemical vessel staining and vessel image analysis were investigated in the femoral heads after the operation. [Results] Gross histology, a great deal of new tissue elongated into the infilling areas on the 3rd week. On the 12th week, the bone trabecula was distributed in all over the infilling areas. Vessel count, a large quantity of new vessel proliferated around the infilling material. Its quantity continuously increased on the 6th week and 12th week. Vessel surface area, the ratio of vessel cross section and the microfield of vision was 18.27% on the 3rd week. This ratio continuously increased on the 6th week and 12th week. [Conclusion] TAACB, a kind of fine carrier of growth factors, is suitable for new vessel growth. AACB/BMP/bFGF compsite has a strong activity of revascularization effect and can improve the ability of bone repair in the osteoneerotic areas of the femoral head. This therapy may become a choice of treatment for femoral head necrosis in the future.
