下颌中切牙牙槽骨吸收的三维有限元应力分析

摘  要:目的:研究下颌前牙正常牙周组织和两侧中切牙三种不同程度牙槽骨吸收后,轴向载荷和斜向载荷作用下,牙体组织的应力变化情况。方法:利用螺旋CT扫描的方法,采用图像处理和有限元分析软件建立321|123及支持组织的三维有限元模型;适当修改牙槽骨颈缘部位的形状,分别模拟中切牙牙槽骨2mm、4mm、6mm吸收,建立3种不同牙槽骨高度的三维有限元模型;在轴向载荷和斜向载荷下计算出牙体颈缘部位的VoniMses应力。结果:①在轴向载荷下:随着中切牙牙槽骨吸收的程度加重,其唇侧、舌侧应力极值逐渐增大,唇侧应力差值在6.8Mpa,舌侧应力差值在1.8Mpa。②在斜向载荷下:随着中切牙牙槽骨吸收的程度加重,其唇侧、舌侧应力值逐渐增大,唇侧应力差值在48Mpa,舌侧应力差值在19Mpa。结论:无论是轴向加载或是斜向加载,中切牙的应力值都随着牙槽骨高度的降低而增加,侧切牙的应力极值变化不明显,尖牙的应力极值较均匀。通过本实验给临床提供可靠的参考依据,并具有指导意义,为下一步联冠修复的优化选择奠定了良好基础。Objective To study the stress distribution situation in the mandibular anterior teeth under the condition of vertical loading and oblique loading, when its periodontal supporting tissues and central incisor are absorbed to three different kinds of degree. Methods By the Computed Tomographic (CT) scanning technology, the three - dimensional finite element model of 3211123 is established with the software for processing image and the Ansys. Three models with different periodontal supporting tissues are also established by changing the form of alveolar bone in the cervical region to simulate the 2mm,4mm,6mm loss of it. Finally to calculate the Von Mises stress in cervical region under the vertical loading and the oblique loading. Results ①Under the vertical loading on central incisor, the maximum of its stress distribution on labial side and lingual side is increased with the decrease of alveolar bone's height, the range of its value on labial side is 6.8 Mpa, the one on lingual side is 1.8 Mpa.②Under the oblique loading on central incisor, the maximum of its stress distribution on labial side and lingual side is increased with the decrease of alveolar bone's height, the range of its value on labial side is 48Mpa,the one on lingual side is 19Mpa. Conclusion Whatever the vertical or oblique stress is loaded on central incisor, the stress distribution of central incisor is increased with the decrease of alveolar bone's height. The one of lateral incisor is steady,the one of tusk has a steady maximum.The experiment brings the reliable reference to clinical treatment and facilitates the further research about the restoration with linked crown.
