帕金森病患者事件相关电位和认知功能障碍及其相关因素分析

摘　　要：目的探讨帕金森病(PD)患者事件相关电位变化和认知功能改变,并对其相关因素进行分析。方法应用韦氏智力、记忆量表对45例PD患者(PD组)和30例健康体检者(对照组)进行智力及记忆功能测试,并检测其事件相关电位各波的潜伏期(PL)和P3的波幅(Amp)。结果PD组P3的PL较对照组显著延长,Amp较对照组显著降低。脑萎缩、应用抗胆碱药治疗时间与P3的PL呈正相关,与P3的Amp呈负相关。PD组的语言智商(VIQ)、操作智商(PIQ)、全智商(FIQ)、短时记忆、瞬间记忆分值显著低于对照组。年龄、运动功能障碍、脑萎缩与VIQ、PIQ、FIQ、记忆商呈负相关;受教育年限与VIQ、PIQ、FIQ呈正相关,应用抗胆碱药治疗时间与VIQ呈负相关。结论PD组患者认知功能障碍在量表测试方面表现为VIQ、PIQ、FIQ、短时记忆、瞬间记忆障碍,且与年龄、运动功能障碍、脑萎缩以及抗胆碱药应用时间相关。Objective To explore event-related potential（ ERP） and the cognitive impairment in patients with Parkinson＇s disease （PD） and analyze their correlative factors.Methods The latent period（PL） of all waves of ERP and amplitude of P3 were measured by using WAIS-RC and WMS-RC in 45 eases of PD and 30 cases of control. Results Measurement of ERP revealed that the PL of P3 were significantly longer and the Amp of P3 were significantly lower in PD group than in control group. The brain atrophy and the time of anticholinergic treatment had positive correlations with the PL of P3 and negative correlations with the Amp of P3- Motor dysfunction had positive correlation with the PL of P3 and negative correlation with the Amp of P3. VIQ, PIQ, FIQ, the seores of short-term memory and immediate memory in PD group were significantly lower than those in control group. Motor dysfunction, brain atrophy, the age of the patients had negative correlations with VIQ, PIQ, FIQ and MQ. The duration （years） of education had positive correlation with VIQ,PIQ and FIQ. The time of anticholinergic treatment had negative correlation with VIQ. Conclusions The cognitive impairment in patients with PD showed the disorders of VIQ, PIQ, FIQ, short-term memory ar,d immediate memory. In WAIS-RC and WMS-RC examinations,it showed close relation with the age of patients,motor dysftmction, brain atrophy,and the time of anticholinergic treatment.
