早期食管癌的病理分型与其影像诊断的关系(附11例报告)

摘　　要：目的探讨早期食管癌的病理分型与内镜、钡餐造影和计算机体层摄影(CT)的影像学诊断结果的关系。方法回顾性分析11例早期食管癌的内镜、钡餐造影、CT和手术前后的病理检查结果。结果11例均为早期食管癌,鳞癌10例,腺癌1例。病理分型:4例乳头型,4例斑块型,2例糜烂型,1例平坦型(隐伏型)。术前内镜检查发现病变10例,1例平坦型未发现。食管钡餐造影显示病变7例,其中隆起型、斑块型各3例,另1例为平坦型。CT检查示食管壁异常5例,其中隆起型、斑块型各2例,另1例为平坦型。结论内镜发现早期食管癌优于食管钡餐造影和CT。而食管钡餐造影和CT又为内镜和临床手术治疗提供更进一步的诊断依据。Objective To investigate the relationship between pathological type and the diagnostic outcomes of endoscopy, barium meal, CT of early esophagus carcinoma. Methods The pathological results of 11 patients who underwent the examinations of endoscopy, barium meal, CT and operation, were retrospectively analyzed. Results Of 11 patients with early carcinoma of esophagus confirmed by operation and pathology, there were 10 cases of squamous carcinoma and one case of adenocarcinoma. For pathological types, there were 4 cases of papillary type, 4 cases of plaque type, 2 cases of ulcerous type and one case of occult type. Preoperative endoscopy displayed the lesions in 10 cases and no lesion in one case of occult type. Barium meal revealed 7 cases of lesions, including 3 cases of protrusion type and 3 cases of plaque type, and one case of occult type. CT examination indicated there were 5 cases of abnormal esophageous wall, including 2 cases of protrusion type and plaque type in each, and one case of occult type. Conclusion Endoscope is better than barium meal and CT in detecting early carcinoma of e sophagus, but barium meal and CT can offer advanced diagnostic basis for clinical practice, and can make up the deficiencies of endoscopy.
