摘  要:目的探讨臂踝脉搏波传导速度(baPWV)与脑出血、脑梗死的相关性。方法对111例急性脑出血、脑梗死患者临床资料进行回顾性分析,按病变性质分为脑出血组(50例)和脑梗死组(60例),对照组46例为非脑出血、脑梗死患者,比较分析不同组的baPWV值,分析颈动脉粥样硬化发生率与baPWV之间的关系及脑出血、脑梗死的危险因素。结果(1)baPWV在脑出血组、脑梗死组与对照组之间差异有统计学意义(P<0.01)。(2)有颈动脉粥样硬化的脑出血、脑梗死患者,其baPWV值明显增高(均P<0.01)。(3)在所有危险因素中,baPWV是脑出血、脑梗死最敏感危险因素(均P<0.01),高血压是脑出血、脑梗死的较敏感危险因素(均P<0.05),年龄、吸烟也是脑梗死的较敏感危险因素(均P<0.05)。结论baPWV是反映大动脉弹性的参数,是与脑出血、脑梗死发生相关的一个重要和独立的因素,可作为发生脑血管意外的一个危险信号,以指导早期干预。Objective To Einvestigate the relationship of brachial ankle pulse wave velocity (baPWV) with intracerebral hemorrhage and cerebral infarction. Methods Clinical data of 110 patients with acute intrac- erebral hemorrhage (n=50) and cerebral infarction (n=60) were retrospectively analyzed. Forty six non-intracere- bral hemorrhage and cerebral infarction cases were also studied as controls, baPWV measurement was performed in all patients and controls. The correlation of baPWV with disease status and carotid atherosclerosis was analyzed. Results There was a significant difference in baPWV between the intracerebral hemorrhage and cerebral infarction (P〈0.01). baPWV was significantly higher in intracerebral hemorrhage and cerebral infarction patients with carotid atherosclerosis (P〈0.01). baPWV was the most sensitive risk factor for intracerebral hemorrhage and cerebral infarction (P〈0.01). Hypertension was the more sensitive risk factor for intracerebral hemorrhage and cerebral infarction (P〈0.05). Smoking and age were the more sensitive risk factors for cerebral infarction (P〈 0.05). Conclusion baPWV may be the simplist parameter reflecting large artery elasticity and closely related to severity of intracerebral hemorrhage and cerebral infarction.
