摘 要:目的观察维吾尔族正常眼压性青光眼患者与对照组的血液流变学的指标差异,对正常眼压性青光眼的发病机制进行探讨。方法维吾尔族正常眼压性青光眼患者37例,维吾尔族白内障对照组患者40例,汉族正常眼压性青光眼对照组患者40例。3组年龄、性别无显著性差异。分别测定全血粘度(包括高切、低切)、血浆粘度、红细胞压积、纤维蛋白原。其中全血粘度和血浆粘度采用毛细血管粘度计测定,红细胞压积采用离心沉淀法测定,纤维蛋白原采用双缩脲法,用721分光光度计测定。结果维吾尔族正常眼压性青光眼组的全血粘度、血浆粘度、红细胞压积、纤维蛋白原较维吾尔族白内障对照组有显著的升高(P<0.05),较汉族正常眼压性青光眼对照组有轻微的升高,但无统计学意义(P>0.05)。结论正常眼压性青光眼患者多伴有高粘滞血症,其原因可能是血浆粘度和红细胞压积的增高,其中维吾尔族患者较汉族为重,但在种族因素上无明显差异。血液流变学因素可能参与了正常眼压性青光眼的发病机制,是青光眼视神经损害的潜在危险因素。Objective To assess the role of the blood rheology indices in pathogenesis of normal tension glaucoma by analyzing the level difference of the blood rheology indices between Uigur patients with normal tension glaucoma and control groups.Methods The blood rheology indices(including blood viscosity, plasma viscosity, hematocrit and fibrinogen)were determined in 37cases of Uigur normal tension glaucoma patients,40 cases of Han nationality normal tension glaucoma patients and 40 cases of Uigur cataract patients.The blood viscosity and plasma viscosity were determined by capillary viscosimenter.The hematocrit was determined by centrifugal sedimentation. The fibrinogen were determined by biuret method,using speotrophotometer.Results The blood rheology indices (including blood Viscosity, plasma viscosity,hematocrit and fibrinogen) in Uigur normal tension glaucoma group were significantly higher than those of Uigur cataract group(P〈0.05).They were slightly higher than those of Han nationality normal tension glaucoma group.There were no significant difference between Uigur normal tension glaucoma group and Han nationality normal tension glaucoma group (P〉0.05).Conclusion The alteration of the blood rheology indices in patients with normal tension glaucoma supports our hypothesis that patients with normal tension glaucoma have blood hyperviscosity.The increase of plasma and fibrinogen may be the reason for blood hyperviscosity. It did not significantly correlate with races of Uigur and Han nationality.The factors of blood rheology participated in the pathogenesis of normal tension glaucoma, which is a potential risk factor of glaucomatous optic neuropathy.
