恶性肿瘤患者生存质量的调查研究

摘　　要：目的 调查临床肿瘤患者的生存质量情况及其影响因素，为卫生部门提高其生存质量提供依据。方法 采用面访问卷调查方法，通过填写生存质量评价量表，调查了四川省各市州2001年1月～2002年7月确诊的639名恶性肿瘤患者的生存质量及其影响因素。结果 恶性肿瘤患者总体生存质量较低，评分为54．40±10．95。管理职业者与心理状态呈正相关（P〈0．05）；费用报销比例高、文化程度高、管理职业者、男性、手术效果好与家庭社会功能呈正相关（P〈0．05，P（O．01），这些因素有助于促进患者家庭社会功能的恢复；手术效果好、采用化疗及放疗与一般躯体功能呈正相关（P〈0．05），可以促进患者躯体功能的恢复。结论 恶性肿瘤患者生存质量问题较普遍，医务人员应重视长期生存者的全面康复，针对患者开展肿瘤学、心理学及社会医学等多学科的干预和健康教育。Objective To investigate the quality of life for clinical patients with malignant tumors and the influencing factors so as to provide health departments with basis for improvement. Methods Using methods of interviews and questionnaires, investigations were made on the quality of life for 639 patients with malignant tumors diagnosed between January 2001 and July 2002 in the various cities and regions of Sichuan and the influencing factors by means of filling out quality of life assessment scales. Results The quality of life for patients with malignant tumors was in general pretty low, scoring 54. 40 ± 10. 95. Careers in management were positively related to psychological status （P〈0. 05）; a high rate of reimbursed expenses, high educational background, careers in management, male gender, and good surgical results, which were positively related to familial and social functioning （P〈0. 05）, were beneficial to the recovery of familial and social functioning; good surgical results and the use of chemotherapy and radiation therapy, which were positively related to general somatic functions （P〈0. 05）, were beneficial to the recovery of somatic functioning. Conclusion Low quality of life for patients with malignant tumors is a quite common problem. Medical workers ought to pay attention to the overall rehabilitation of long-term survivors and conduct relevant interventions and health education involving multidisciplines like oncology, psychology and sociomedicine.
