摘 要:目的研究亚低温对脑出血后水通道蛋白4(AQP4)表达及脑水肿的影响,探讨亚低温对出血性脑水肿的作用机制。方法在大鼠苍白球注射胶原酶制作脑出血模型,用冰块降温及白炽灯照射加温的方法调节体温;应用免疫组化方法检测脑组织AQP4表达,采用干湿重法观察脑含水量的动态变化。结果脑出血模型大鼠病灶侧脑含水量、血肿周围AQP4的表达水平明显高于假手术组(均P<0.001);各个时间点亚低温组大鼠病灶侧脑含水量以及血肿周围AQP4的表达水平均明显低于对照组(P<0.05~0.01);AQP4表达水平与脑含水量呈正相关(r=0.977,P<0.001)。结论亚低温能明显减轻脑出血后脑水肿及AQP4的表达,亚低温可能通过抑制AQP4的表达而减轻脑水肿。Objective To study the effect of mild hypothermia on aquaporin 4 (AQP4) expressions and cerebral edema after intracerebral hemorrhage and the mechanisms of mild hypothermia on hemorrhagic brain edema. Methods The model of intracerebral hemorrhage (ICH) was established by infusing collagenase into right globus pallidus of the rat. The temperature was regulated by ice cubes and daylight lamp, which was modulated to an appropriate distance to the animals. Immunohistochemistry staining was used to detect the cerebral AQP4 expressions. Dynamic change of brain edema was observed through wet-dry weighing method. Results The brain water content (BWC) of injured side and level of AQP4 expression in peribematom tissue in ICH rats were obviously higher than those of the sham group ( all P 〈0. 001 ). In the mild hypothermia group, the BWC and AQP4 expression level were obviously lower than those of the control group at every time point (P 〈 0.05 - 0.01 ). The level of AQP4 expression was positively correlated with BWC ( r = 0. 977, P 〈 0. 001 ). Conclusion Mild hypothermia may relieve cerebral edema after ICH and inhibiting AQP4 expressions may be related with its mechanism.
