胃黏液癌黏液成分的判定与浸润转移的相关性研究

摘　　要：目的探讨胃黏液癌黏液的性质及胃黏液癌中nm23与MUC1、MUC2蛋白表达与浸润转移的关系。方法采用黏液组织化学方法对57例胃黏液癌进行了AB/PAS/HID-AB检测,同时采用免疫组织化学技术分别检测了nm23、MUC1、MUC2蛋白的表达。结果①胃黏液腺癌以肠型腺癌为主,胃印戒细胞癌以胃型腺癌为主。其黏液成分与胃黏液癌的浸润转移无相关。②胃黏液癌中,nm23、MUC1、MUC2表达率分别为36.84%、64.91%、56.14%。nm23、MUC1的表达与浸润深度、有无淋巴结转移有关(P<0.05),而与黏液的性质无关(P>0.05),MUC2的表达与组织学类型及黏液性质有关(P<0.05)。结论胃黏液腺癌的组织学发生是来自肠上皮的化生;胃印戒细胞癌的组织学发生是来自胃腺上皮。AB-PAS,HID-AB和MUC2的检测可鉴定胃黏液癌黏液的性质。nm23、MUC1的表达,对判断胃黏液癌的预后具有参考价值。Objective To invest the mucus＇ property of the gastric mucinous cancer and the expression of nm23, MUC1, MUC2 of the gastric mucinous cancer, as well as the relationship between the gastric cancer invasive and metastasis. Methods Fifty-seven samples of gastric colloid cancer were examined by AB/PAS and HID/AB. The expression of nm23, MUC1, MUC2 were detected by SP immunohistochemistry. Results ①The majority of gastric municinous adenocarcinoma samples were intestine type; the majority of signetfing-cell carcinoma samples were gastric type. There was no correlation between the property of mucus and the invasive metastasis of gastric mucinous cancer. QThe expression percentage of nm23, MUC1, MUC2 were 36.84%, 64.91% ,56.14% ,respectively.Expression of nm23,MUC1 were relevant to the invasive depth and the lymph node metastasis （ P 〈 0.05）, however, they were not relevant to the property of mucus（ P 〉 0.05）, and the expression of MUC2 was relevant to the histological type and the property of mucus（ P 〈 0.05）. Conclusion The histologic origin of gastric mucinous adenoma was intestinc epithelium cells; the histologic origin of gastric ring-cell carcinoma was gastric gland epithelinm cells. The detection of AB-PAS, HID-AB and MUC2 could be used for evaluating the character of gastric mucinous carcinomas colloid. The expression of nm23, MUC1 could be useful in judging the gastric mucinous carcinoma＇ s prognosis.
