眼部髓上皮瘤临床特征分析

摘　　要：目的:分析眼及眼眶髓上皮瘤的临床表现、影像学特征、病理组织学特点、治疗及预后。方法:对1990/2005年经病理检查证实的8例8眼眼眶髓上皮瘤的临床、影像及病理资料进行回顾性分析。结果:眼内髓上皮瘤首诊表现有白内障、青光眼及前葡萄炎等,眶内表现为眼球突出及运动障碍。B超检查形态多样,内回声少或中等;彩超检查血流信号少;UBM检查可见睫状体区实性肿物。CT检查见软组织肿物。8例均行手术治疗,4例术后辅助放化疗,随访中无1例转移及死亡。结论:髓上皮瘤是临床上少见的眼科肿瘤,新生睫状体假膜为其特征性表现,常继发新生血管性青光眼及白内障。影像学检查有助于髓上皮瘤的早期诊断,眼球摘除或局部切除加近距离放疗疗效佳,其预后较好。AIM： To analyze the clinical, imaging, pathological features of medulloepithelioma and investigate the treatment and prognosis of medulloepithelioma. METHODS： Histories, imaging and pathological findings of eight cases were reviewed as a retrospective study. RESULTS： Clinically intraocular medulloepithelioma could induce cataract, glaucoma, or uveitis. The orbital cases caused proptosis and oculomotor palsy. B-scan showed the polymorphic masses with low to medium echogenicity. UBM of one case showed a mass at ciliary body, with no distinctive findings. CT showed soft tissue neoplasma. Eight patients were treated by the operations, four cases underwent branchytherapy after the operation. No case had distance metabasis or death. CONCLUSION： Medulloepithelioma is a rare neoplasm, which tend to cause secondary neovascular glaucoma, cataract and neoplastic cyclitic membrane. The finding of an unusual cyclitic membrane possibily suggests diagnosis of the medulloepithelioma. Echographic features conduce to early diagnosis. Enucleation and brachytherapy after local resection are effective in treating medulloepithelioma, with good prognosis.
