摘　　要：目的利用彩色多普勒超声研究B inswanger病(BD)患者颈动脉的形态学及血流动力学变化。方法探查42例BD患者和40例年龄匹配、伴随疾病相似的对照组双侧颈总动脉(CCA)、颈内动脉(ICA)与颈外动脉(ECA)粥样斑块声像图特点、颈总动脉壁内中膜厚度(IMT)以及收缩期峰值流速(PSV)、舒张末期流速(EDV)、阻力指数(R I),并将两组进行比较。结果①BD组颈动脉粥样硬化斑块的检出率较对照组增高(P<0.05),两组均以硬斑、扁平斑多见。②颈动脉狭窄两组均以轻度为主,但BD组中、重度狭窄例数明显高于对照组(P<0.01);BD组IMT较对照组增厚,两组差异有显著性(P<0.05);③BD组PSV及EDV均较对照组减慢(P<0.05)。结论BD的发病与颈动脉粥样硬化导致脑部血流低灌注有关,颈动脉彩色多普勒探查在评价本病的发病机制和病程进展中有重要意义。Objective To study the changes of morphology and hemodynamics of the carotid artery with color Doppler flow imaging （CDFI） in patients with Binswanger＇s disease （BD）. Methods In 42 patients with BD （BD group） to compare with 40 patients of similar age and complications, without symptoms and signs of ischemic heart disease or ischemic encephalopathy （ control group）, CDFI was used for quantification of the uhrasonographic features of atherosclerotic plaques in common carotid artery （CCA）, internal carotid artery （ICA）, external carotid artery（ ECA）, intima-media thickness（ IMT）, and the hemodynamics parameter in CCA. Results 1. The incidence rate of atherosclerotic plaques in BD group was significantly higher than that in control group （P 〈0. 05）. 2, The majority in two groups showed mild degrees of carotid stenosis （ 〈 50% ） , but intermediate and severe degrees （ 〉 50% ） observed in BD group were more frequent than those in control group（P 〈 O. Ol ）. In contrast to control group, IMT of CCA in BD group was significantly thicker （ P 〈0.05）. 3. PSV and EDV in CCA were significantly reduced in BD group compared with those in control group （ P 〈0.05 ）. Conclusions The occurrence of Binswanger＇s disease was highly associated with carotid atherosclerotic. Color Doppler flow imaging of carotid artery is of great value in clinical application for Binswanger＇ s disease.
