综合方法防治神经缺血再灌注的临床研究

摘  要:[目的]观察甘露醇、磷酸川芎嗪和高压氧治疗神经损伤后缺血再灌注的临床效果。[方法]选取神经损伤病人98例,按入院顺序随机将病人分为对照组和治疗组,治疗组在与对照组应用维生素类药物治疗、理疗和功能锻炼相同治疗基础上,采用甘露醇、磷酸川芎嗪和高压氧治疗,通过肌电图检测运动神经动作电位,放射免疫法测定超氧化物歧化酶(SOD)、丙二醛(MDA)的含量变化,以及神经功能恢复情况。[结果]治疗组治疗后运动神经动作电位的波幅、运动传导速度明显加快,SOD含量增加,MDA含量减低,神经功能综合评价优于对照组(P<0.05)。[结论]应用甘露醇、川芎嗪和高压氧综合治疗周围神经损伤后缺血再灌注,能从分子水平、电生理和功能上修复和改善损伤神经。Objective: To observe the clinical effect of mannitol, Ligustrazine sulfate and hyperbaric oxygen to treat patients with ischemia reperfusion after nerve injury. Method: A total of 98 nerve injury patients were selected and divided into test and control group randomly. Patients in test group were given mannitol, Ligustrazine sulfate and hyperbaric oxygen based on administrating of vitamins, physical treatment and functional exercise, which only provided in control group cases. Then the action potential of motor nerve was measured by electromyogram. The alternation of both blood superoxide dismutase (SOD) and malonaldehyde (MDA) contents were detected by radioimmunoassay and the recovery of nerve function were observed as well. Result: The wave amplitude of the action potential of motor nerve in patients after treatment was enlarged markedly than that of before treatment. The motor conduction velocity of patient after treatment was faster than that of before treatment. The blood SOD level was higher and MDA concentration was lower in patients after treatment than that of before treatment. In terms of the recovery of nerve function, patients of test group were superior to those in control group (P 〈 0.05). Conclusion: The ischemia reperfusion after peripheral nerve injury of patients can be fixed and improved in electrophysiologieal and functionally and at molecular level by using mannitol, Ligustrazine sulfate and hyperbaric oxygen comprehensively.
