失语症的常用评价方法

摘　　要：目的:脑卒中后患有失语症的患者比例较多,致使患者交流困难,甚至丧失社会交往能力,严重影响患者的生活质量。资料来源:应用计算机检索Ovid1980-01/2004-01期间与失语症评价方法相关的文章,检索词“aphasia,estimate”,“aphasia,evaluate”,“aphasia,estimationmethod”,并限定语种为English;同时也检索中国医学期刊网1980-01/2004-01期间相关的文章,检索词为“失语症,评估”,“失语症,检测”,限定语言种类为汉语。资料选择:对资料进行初审,选取有关失语症评定方法的相关文献,然后筛除明显不随机临床试验的研究,对剩余的文献查找全文,进一步判断是否为随机对照临床研究。无论是否为单盲,双盲或非盲法。排除重复性研究。资料提炼:共收集到21篇关于失语症评价方法的相关文献,排除8篇重复的同一研究试验。采用其余13篇进行综述。资料综合:目前国内外已经有许多失语症的评估方法,国外常用的评价方法有:TheTokenTest;波士顿诊断性失语症检查;西部失语症检查;双语失语检查法;失语症的标准语言试验;日常生活交流能力检查;HenryHead’sTest;WeisenburyandMcBride’sBattery;明尼苏达失语鉴别诊断;Porch交流能力指数;功能交流概貌测定;国内常用的评价方法有:汉语失语成套测验;北京医院汉语失语症检查;中国康复研究中心汉语标准失语症检查。结论:失语症的评估方法国内外有很多,如何依据患者的语言障碍程度,选择失语症的评估方法是判断失语症类型及制定失语症患者语言康复计划的关键问题。OBJECTIVE： Many stroke patients suffer from aphasia, which make them unable to communicate with others, even deprive their ability of social communication and severely damage their life quality. DATA SOURCES： A computer-based search for articles related to the estimation methods of aphasia from January 1980 to January 2004 was conducted in Ovid with the key words of ＂aphasia, estimate＂, ＂aphasia, evaluate＂ and ＂aphasia, estimation method＂, and the language was limited to English. Meanwhile, relevant Chinese articles from January 1980 to January 2004 were searched in China medical network with the key words of ＂aphasia, estimate＂ and ＂aphasia, evaluate＂. STUDY SELECTION： Data were checked in the primary trial, and literatures related to the estimation methods of aphasia were selected, while those with obvious nonrandomized clinical trials were exclude. The remaining papers were looked for the full-text to judge whether they were randomized control clinical research without considering whether they were single blind, double blind, or non-blind. Repetitive studies were excluded. DATA EXTRACTION： A total of 12 articles related to the estimation methods of aphasia were collected, and 8 repetitive trials were excluded. The rest 13 enrolled articles were reviewed. DATA SYNTHESIS： By now, there are multiple estimation methods of aphasia at home and abroad, and common ones abroad are： the token test, Boston diagnostic aphasia examination, the western aphasia battery （WAB）, the bilingual aphasia test, standard language test of aphasia, examination of ability in daily life, Henry Head＇s Test, Weisenbury and McBride＇s Battery, Minnesota aphasia examination, Porch index in communication and general determination of functional communication. The estimation methods at home are： aphasia battery of Chinese, aphasia battery examination of Beijing hospital and clinical rehabilitation research center aphasia examination. CONCLUSION： There are many ways of aphasia estimation at home
