卵巢囊性成熟性畸胎瘤的B超特点及误诊原因分析

摘　　要：目的分析卵巢囊性成熟性畸胎瘤超声诊断及鉴别特点,以减少超声误诊率。方法选择卵巢囊性成熟性畸胎瘤89例,平均年龄38岁。行腹部及经阴道超声检查,探查瘤体大小及内部回声特点,并与手术病理做对照。结果本组89例,肿物92枚,左侧46例,右侧40例,双侧3例。其中卵巢囊性成熟性畸胎瘤声像图呈囊实性65枚,类实性18枚,囊性9枚;误诊11枚,其中囊性9枚,囊实性1枚,类实性1枚。结论卵巢囊性成熟性畸胎瘤囊内呈中高回声,若形成特征性的改变则是超声诊断的依据;若缺乏其特征性,则是误诊的主要原因。超声鉴别诊断有一定作用,但需结合临床及病理。Objective To study the ultra senographic features of mature cystic ovarian teratomas in order to reduce ultrasonographic misdiagnosis. Methods 89 cases（92 masses）with mature cystic ovarian teratomas confirmed by operation and pathology who were examined by ultrasonography were studied. Results The ultrasonographic images of 89 cases （92 masses）with mature cystic ovarian teratomas displayed three types：cystic and solid （65 masses）, false solid（18 masses） and cyst（9masses） type. 11 masses were misdiagnosed including 9 cyst masses of, 1 cystic and solid mass, 1 false solid mass. Conclusions The accuracy of ultrasonography in diagnosing mature cystic ovarian teratomas is based on the sonographic characteristic presentation otherwise it can cause misdiagnosis. There is some value in differential diagnosis by ultrasenograthy, but need to combine with clinical data and pathology.
