摘  要:目的 研究低氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)及胰岛素样生长因子-Ⅰ(IGF-Ⅰ)蛋白在子宫内膜癌中的表达及其意义,并探讨HIF-1α与IGF-Ⅰ及VEGF蛋白表达之间的关系.方法 应用免疫组织化学PicTureTM方法检测HIF-1α、VEGF、IGF-Ⅰ蛋白在42例子宫内膜癌、10例子宫内膜不典型增生及20例正常子宫内膜组织中的表达情况,结合临床分期、病理分级进行分析.结果 HIF-1α、VEGF、IGF-Ⅰ蛋白在正常子宫内膜组织、子宫内膜不典型增生组织及子宫内膜癌组织中阳性表达率比较,差异有显著性(χ^2=22.094~25.261,P〈0.01).HIF-1α蛋白在子宫内膜癌组织中的阳性表达率Ⅰ期为42.11%,Ⅱ期为81.82%,Ⅲ期为83.33%,三者比较有显著性差异(χ^2=7.435,P〈0.05);HIF-1α蛋白的表达与病人发病年龄、病理分级无关(χ^2=0.036、0.634,P〉0.05).VEGF蛋白在子宫内膜癌组织中阳性表达率Ⅰ期为36.84%,Ⅱ期为81.81%,Ⅲ期为75.00%,三者比较差异有显著性(χ^2=6.578,P〈0.05);VEGF蛋白的表达与病人发病年龄、病理分级无关(χ^2=1.879、0.097,P〉0.05).子宫内膜癌组织中IGF-Ⅰ蛋白阳性表达率与病人发病年龄、临床分期及病理分级无关(χ^2=0.190~0.791,P〉0.05).子宫内膜癌组织中HIF-1α与VEGF蛋白表达呈显著正相关(r=0.469,P〈0.01);IGF-Ⅰ与VEGF蛋白的表达呈显著的正相关(r=0.452,P〈0.01);HIF-1α与IGF-I蛋白的表达无相关性(r=0.209,P〉0.05).结论 子宫内膜癌组织中HIF-1α、VEGF、IGF-I蛋白参与了子宫内膜癌的发生发展.子宫内膜癌组织中的HIF-1α、IGF-Ⅰ蛋白表达与VEGF呈正相关,IGF-Ⅰ蛋白与HIF-1α的表达无相关性,提示HIF-1α与IGF-Ⅰ可能通过上调VEGF蛋白的表达,促进肿瘤的血管生成而促进子宫内膜癌的发生.Objective To study the expression of hypoxia inducible factor-1 alpha (HIF-1α), vascular endothelial growth factor (VEGF) and insulin-like growth factor- Ⅰ (IGF- Ⅰ ) in endometrial carcinoma and to investigate the relationship between HIF-1α, VEGF and IGF- Ⅰ . Methods lmmunohistochemistry PicTureTM method was used to detect the expression of HIF-1α, VEGF and IGF- Ⅰ protein in 42 cases of endometrial adenoearcinoma, 10 cases of atypical hyperplasia of endometrium and 20 cases of normal endometrium. Results HIF-1α, VEGF and IGF- Ⅰ were expressed in endometrial adenocarcinoma tissues, and the positive rales in atypical hyperplasia were 20%, 30% and 10%, respectively. The positive rates in endometrial carcinoma were 64.29%, 61.90% and 59.52%. In contrast, normal endometrium did not express HIF-1α,VEGF or IGF- Ⅰ The expressions of HIF-1α,VEGF and IGF- Ⅰ protein in endometrial adenocarcinoma were significantly higher than in normal endometrium tissues (χ^2=22. 094-25. 261 ,P〈0.01 ). The positive rate of HIF-1α was strongly associated with the clinical stage. The expression rates of HIF-1α protein in stages Ⅰ, Ⅱ and Ⅲ were 42.11%, 81.82% and 83.33%, respectively (χ^2=7. 435,P〈0. 05 ). There was no relationship between HIF-1α protein expression with age or histological grade (χ^2=0. 036,0. 634; P〉0.05). The positive rate of VEGF was strongly associated with the clinical stage. The expression rates of VEGF protein in stages Ⅰ , Ⅱ and Ⅲ were 36.84%, 81.81% and 75.00%(χ^2=6. 578,P〈0.05 ). There was no relationship between VEGF protein expression and age or histological grade (χ^2= 1. 879,0. 097; P〉0.05). There was no relationship between IGF- Ⅰ protein expression and age or clinical stage or histological grade (χ^2= 0. 190-0.791, P;〉0.05).
