肾上腺髓质素在胰岛素瘤患者体内的表达及意义

摘　　要：目的研究胰岛细胞瘤患者体内肾上腺髓质素的表达与分布,其表达是否具有特异性。方法9例手术治疗的胰岛素瘤患者,通过放免法检测循环中的肾上腺髓质素;免疫组化法确定肾上腺髓质素在肿瘤组织中的定位与分布。结果胰岛素瘤患者体内的肾上腺髓质素浓度比正常对照组显著增高。术后患者血浆肾上腺髓质素浓度从(6.3±2.9)fmol.ml-1下降到(2.9±1.7)fmol.ml-1(P<0.001)。免疫组化显示肾上腺髓质素被专一地定位于肿瘤细胞中。在胰腺基质、实质和主要的胰腺导管中肾上腺髓质素表达呈阴性。结论循环中的肾上腺髓质素在胰岛素瘤中的表达是增高的,且这种增高与肿瘤的显型相关。Objective To investigate adrenomedullin secretion expression and the particularity in patients with insulinsecreting islet cell tumours. Methods The study was performed in nine patients with surgically treated insulinoma. Circulating adrenomedullin was assed using a specific radio y and its localization and distribution in the tumour were determined by means of immunohistochemistry. Results Adrenomedullin concertrations were significantly greater in patients with insulinoma than in controls. After operation, plasma adrenomedullin decreased form 6.3 ± 2.9 fmol/ml to 2.9 ± 1.7 fmol/ml （ P 〈 0. 001 ）. Immunoreaotive adrenomdeullin was locaoized exclusively in rumours cells, whereas stroma, surrounding pancreas parenchyma and major ducts were negative for the peptide. Conclusion Circulating adrenomedullin is increased in insulinoma and the increase is related to the neoplastic phenotype.
