视网膜母细胞瘤术后眶颧畸形的综合整复

摘　　要：目的推荐一个视网膜母细胞瘤眼球摘除及放射治疗后眶颧畸形的综合整复方法。方法采用高密度聚乙烯材料作眶颧重建、羟基磷灰石义眼座眶内植入、颞浅筋膜瓣或颞肌筋膜瓣眶内转移和结膜囊再造等联合手术的方式,对该种畸形进行整复治疗。结果本组35例(35眼)手术均取得了成功,术后患者畸形得到矫正,眶颧部饱满,外形良好,双眼外观基本对称,结膜囊深浅正常,无任何并发症,中远期效果令人满意。结论对视网膜母细胞瘤眼球摘除及放射治疗后的眶颧畸形采用综合整复是一种有效的治疗方法。Objective To observe a method of synthetic plastic surgery for orhitozygomatic deformity and contraction of the eye socket caused by enucleation and postoperative irradiation for treatment of retinoblastoma. Methods Orbitozygomatic deformity reconstruction with high density polyeihylene （Medpor） and a hydroxyapatite （HA） implant for correction of anophthalmos. A prefabricated temporal fascial flap or temporal muscular fascial flap is transposed into the eye socket and a mucous membrane graft or a free skin graft was applied over the temporal fascial flap for eye socket reconstruction. Results Excellent cosmetic results of this method in 35 cases were demonstrated. Deformity of the orbitozygoma was corrected, and a large permanent eye socket was achieved in this operation, no remarkable complications. Conclusions This synthetic plastic surgery is an effective method in treatment of orbitozygomatic deformity and contraction of the eye socket.
