摘  要:目的探讨阿尔茨海默病(AD)与血管性因素之间的关系。方法选取解放军总医院临床诊断可能的AD患者91例,以性别、职业和受教育程度相匹配的认知功能正常老年人155例为对照,以Logistic回归模型分析各种血管性因素与AD的关系。结果单因素分析显示,起搏器心律、贫血、冠心病、慢性阻塞性肺疾病、脑卒中、心律失常、短暂性脑缺血发作、心脏传导阻滞、高脂血症、糖尿病史、婚姻状况及年龄与AD有关,OR值范围为12.83~1.18。多因素分析校正了年龄和婚姻状况的影响后,证实贫血、冠心病和脑卒中史与AD有关,OR值(95%可信区间)依次为4.87(2.12~11.22)、2.94(1.38~6.27)、2.44(1.28~4.64)。结论AD与血管性因素有关,贫血、冠心病和脑卒中史可能是AD的独立危险因素。Objective Some researches showed that vascular risk factors might be related to Alzheimer's disease (AD), but there are still disputes among neurologists. Therefore, it was our hope to explore the possible relationship between AD and vascular factors. Methods A case-control study including 155 cognitive normal controls and 91 possible AD patiems in Chinese PLA General Hospital was con- ducted. The patients and controls were matched for gender, occupation and education level. The relationship between AD and vascular factors was analyzed by Logistic model. The level of statistical significance of differences in single factor and multivariate analysis was P〈0. 05. Results Single factor analysis showed that 12 factors were found to be correlated with AD, namely pacemaker's cardiac rhythm, anaemia, coronary heart diseases, chronic obstructive pulmonary diseases, stroke, arrhythmia, transient ischemic attack, heart block, hyperlipidemia, diabetes mellitus, marriage status and ages. The OR values were 12. 83, 6. 76, 5. 16, 4. 87, 3. 80, 3.30, 2.88, 2. 81, 2. 05, 1.91, 1. 18 and 1.57, respectively (P〈0. 05). Multivariate analysis confirmed that after controlling the influence of ages and marriage status, the history of anaemia, coronary heart diseases and stroke were related to AD. The OR values (95% confidence imerval, 95MCD were 4. 87 (2. 12--11.22), 2.94 (1.38--6. 27) and 2. 44 (1.28--4. 64) in turn, Conclusion AD is associated to vascular factors, and the history of anaemia, coronary heart diseases and stroke may be the independent risk factors of AD. The prevention and treatment of vascular diseases may improve the cognitive status of elder people.
