摘　　要：目的:评价不同手术方式治疗Vater壶腹癌的疗效。方法:回顾性分析116例经手术治疗的Vater壶腹癌患者的临床病理资料及生存状况。结果:局部切除38例,无手术死亡,术后并发症5例(13.16%),术后1年、3年、5年生存率分别为83.52%,66.34%和51.36%。胰十二指肠切除62例,手术死亡1例(1.61%),术后并发症14例(22.58%),术后1年、3年、5年生存率分别为78.62%、60.12%和43.38%。其中早期Vater壶腹癌行胰十二指肠切除术22例,术后并发症4例(18.18%),术后1年、3年、5年生存率分别为85.33%、64.56%和53.27%,行其他姑息手术16例,1年生存率11.12%,无3、5年生存。结论:手术切除是Vater壶腹癌的有效治疗手段,合理地选择手术方式是提高疗效保证手术安全性的关键。Objective： To study the therapeutic effects of patients with Vater＇s carcinoma of ampulla treated by different surgical methods. Methods： The clinical and pathological data of 116 patients treated by the surgical intervention were retrospectively analyzed. Results： A total of 38 cases with Vater＇s carcinoma of ampulla received local resection. The operative mortality was 0% and the morbid- ity of the diseases was 13.16%. The 1-, 3- and 5-year survival rates were 83.52%, 66.34% and 51.36%, respectively. 62 cases underwent pancreatoduodenectomy, the intraoperative mortality was 1.61% and the rate of postoperative complication was 22.58%. Among them 22 was early cases, the intraoperative mortality was 0% and the rate of postoperative complication was 18.18%. The 1-, 3- and 5-year survival rates were 85.33%, 64.56% and 53.27%, respectively. Palliative operation was performed in 16 cases, the 1-, 3- and 5-year survival rates were 11.12%, 0% and 0%, respectively. Conclusion： Surgical excision is an effective method for carcinoma of ampulla of Vater. Local excision is a safe operation with low complication and a good survival rate in carefully selected cases. It is essential to use rational operation in order to improve the results of surgical treatment.
