80例肠梗阻术后回肠末端吻合口瘘原因分析

摘　　要：【目的】探讨回肠末端吻合口瘘的原因和防治方法。【方法】回顾性分析80例绞窄性肠梗阻病人肠管切除术后的临床效果。【结果】行回-回肠吻合组40例，肠瘘20例，其中8例死亡；20例恢复健康。行回-结肠吻合组40例全部恢复健康。【结论】距回盲瓣10-15cm范围内的回肠末端绞窄坏死的处理与其他部位小肠坏死比较，有其特殊性，处理须充分考虑解剖位置和局部血运，采取慎重态度。[Objective]To find out the causes of fistulae of anastomosed distal ileum and the methods of prevention and treatment. [Methods]Eighty cases with strangulated obstruction were studied. Ileoileostomy （40 cases） and ileotransversostomy（40cases） were applied. [Results]There were 20 cases with intestinal leakage and 8 cases died, while the rest 32 cases recovered in the ileoileostomy group. All 40 cases in the ileotransversostomy group recovered. [Conclusion]We must be careful to handle the strangulated ileum obstruction at the distal 10-15 cm segment.
