胃癌合并糖尿病病人围麻醉期血清胰岛素、皮质醇和血糖变化  

The Changes of Serum Insulin, Cortisol and Blood Glucose in the Diabetic Patients with Gastric Cancer during Anesthesia

摘　　要：观察了17例胃癌合并糖尿病病人围麻醉期血清胰岛素、皮质醇和血糖变化,以及术中输入平衡盐液时血浆电解质的变化。结果表明,胃癌合并糖尿病病人采用神经安定-安氟醚麻醉,血清胰岛素和皮质醇围麻醉期没有明显变化,而血糖随着手术时间延长逐渐增高,麻醉后90min和术毕显著高于麻醉前(P<0.01),说明神经安定-安氟醚麻醉虽然能使糖尿病病人术中血清胰岛素和皮质醇平稳,但不能抑制手术应激所致的血糖增高。术中输入平衡盐液血浆电解质无明显改变,认为平衡盐液可补充功能性细胞外液,降低血液粘滞性,改善组织灌流,纠正酸中毒,有利于糖尿病病人恢复。The changes of serum insulin, cortisol, blood glucose and electrolytes were investigated in 17 cases with gastric cancer accompaning diabetes during anesthesia. The results show that the levels of serum insulin and cortisol in the patients with gastric cancer accompaning diabetes were not significantly changed during anesthesia with neu-roleptanalgesia and enflurane inhalation. Blood glucose, however, rose gradually as the operation prolonged, at 90 min. after anesthesia and by the end of the operation, which was increased significantly as compared with that before anesthesia (P<0. 01). There were no obvious changes in plasma electrolytes. Authors believe that Lactate Ringers Solution can complement fumctional extracelluar fluid, reduce blood stickiness,improve tissure irrigation, correct acidosis and promote diabetic recovery.
