内镜下微波灼扩联合探条扩张治疗食管癌/贲门癌术后吻合口狭窄

摘　　要：目的探讨内镜下微波高温凝固联合探条扩张对食管癌/贲门癌术后吻合口狭窄的疗效。方法采用前瞻性研究,90例食管癌/贲门癌术后吻合口狭窄病人随机分为微波组、探条组、联合组各30例,治疗后跟踪观察吻合口狭窄改善情况。结果治疗1周后有效率(CR+PR):微波组为93.3%,探条组为100%,联合组为100%(与前两组比较P>0.05);1个月后微波组为70.0%,探条组为16.7%,联合组为93.3%(与前两组比较分别为P>0.05,P<0.01);3个月后微波组为53.3%,探条组为6.7%,联合组为86.7%(与前两组比较P<0.01)。结论内镜下微波高温凝固联合探条扩张是食管癌/贲门癌术后吻合口狭窄的最佳选择。[Objective] To study the efficacy of microwave tissue coagulation combined with ＂Savary-Gilliard＂ rod dilation via endoscopy in the treatment of anastomotic-stenosis after esophageal and/or gastric cardiac cancer operation. [Methods] 90 cases of anastomotic-stenosis after esophageal and gastric cardiac cancer operation were divided into three groups at random, viz. Microwave tissue coagulation group/the first group, ＂Savary-Gilliard＂ rod dilation group/the second group and combination group/the third group. [Results] The effective rates （CR＋PR） were 93.3% in Microwave tissue coagulation group, 100% in ＂Savary-Gilliard＂ rod dilation group, 100% （compared with the former two groups P 〉0.05） in combination group after one week＇ s treatment. The rates were 70%, 16.7% and 93.3% （compared with the first group P 〈0.05, compared with the second group P 〈0.01） after one month＇ s treatment and 53.3%, 6.7% and 86.7% （compared with the former two groups P 〈0.01） after three month＇ s treatment in the respective groups. [Conclusions] The therapy of microwave tissue coagulation in combination with ＂Savary-GiUiard＂ rod dilation via endoscopy is choice of treatment in anastomotic-stenosis after esophageal and/or gastric cardiac cancer operation.
