摘 要:目的探讨运动神经元病肺功能受损情况及病理生理原因。方法测定54例患者的肺功能并与正常对照组进行比较。结果患者通气功能明显减低(P<0.01),以限制性通气障碍为主要损害占70%以上,小气道受损约占60%,弥散功能受损占48.14%。结论运动神经元病患者肺功能损害以限制性通气障碍为主。其肺功能损害的病理生理是由于支配呼吸肌的神经元进行性减少,呼吸肌出现失神经萎缩,呼吸肌无力而出现一系列肺功能改变。[Objective] To explore the pathophysiological reasons for pulmonary function disturbance in patients with motor neuron diseases (MND). [Methods] General pulmonary function of 54 MND patients was measured and the data were compared with that of the control group. [Results] Test showed that the ventilation function was re- duced remarkably with significant difference (P 〈0.01). 74% of the patients showed restrictive ventilitory disorder, and 60% showed small airway function injuries, and 48.14% showed diffusion function injuries. [Conclusions] The most characteristic pulmonary function change in MND is the restrictive ventilitory disorder. The pathophysiogical mechanism for pulmonary function disturbance is because of the following: the neuron which steers the respiratory muscles gradually decreasing; the respiratory muscles atrophy and respiratory muscles asthenia appearing; a series changes of pulmonary function occurred.
